Verify Your Nursing Credentials Before Applying
CGFNS credential evaluation and an active state nursing license are required before most U.S. host facilities will consider you for a clinical placement. Start the CGFNS process at least six months before your target start date.
Target Hospitals With Active Exchange Visitor Programs
Not every U.S. hospital is registered as a J-1 host site. Search for facilities that explicitly list exchange visitor or international nurse training programs in their clinical education or workforce development departments.
Use Migrate Mate to Find J-1 Compatible Roles
Search for Graduate Nurse roles on Migrate Mate to identify U.S. employers whose open positions align with J-1 Trainee or Research Scholar program requirements, saving you from outreach to facilities with no exchange visitor infrastructure.
Clarify the Two-Year Home Residency Rule Early
Many nursing J-1 exchange visitors are subject to the two-year home residency requirement under INA Section 212(e). Confirm with your designated sponsor whether your country and funding source trigger this obligation before accepting any offer.
Get Your Training Plan Signed Before the DS-2019 Is Issued
Your designated sponsor organization, such as CGFNS for nursing-related categories, requires a detailed training plan outlining your clinical rotations and learning objectives. The host facility must co-sign this plan before the DS-2019 can be issued.
Confirm Host Site Supervision Requirements With Your Sponsor
J-1 Trainee regulations require a qualified U.S. supervisor at the host facility for all clinical hours. Ask the designated sponsor to confirm the host site's supervision structure meets DOL and State Department compliance standards before you sign an offer.

Find Graduate Nurse JobsGraduate Nurse J-1 Visa: Frequently Asked Questions
Which J-1 program category applies to Graduate Nurse positions?
Graduate nurses most commonly enter under the J-1 Trainee category if they hold a nursing degree and have fewer than five years of post-graduation experience. Nurses involved in formal research or curriculum development at an academic medical center may qualify under the Research Scholar category. Your designated sponsor organization determines the appropriate category based on your credentials, the host facility's program structure, and your specific training objectives.
Who sponsors the J-1 visa for a Graduate Nurse role, and how is that different from the hiring hospital?
The J-1 visa sponsor is a U.S. Department of State-designated organization, such as CGFNS or another approved exchange program, that issues your DS-2019 and monitors your compliance throughout the program. The hospital or health system where you work is the host organization, not the visa sponsor. These are two separate relationships. The host facility provides the clinical placement and training environment, while the designated sponsor carries the legal and regulatory responsibility for your J-1 status.
How can I find U.S. hospitals that support J-1 exchange visitors for nursing roles?
Use Migrate Mate to search for Graduate Nurse positions at U.S. employers whose roles are compatible with J-1 program requirements. Not all hospitals maintain active exchange visitor programs, so filtering for J-1 compatible opportunities early avoids time spent pursuing facilities that have no established host site relationship with a designated sponsor organization.
Does the two-year home residency requirement apply to Graduate Nurses on a J-1 visa?
It depends on your country of nationality, country of last legal residence, and whether your training is funded by a U.S. government or home-country government program. Many nursing exchange visitors are subject to the two-year home residency requirement under INA Section 212(e), which means you must return to your home country for two years before changing to most other U.S. visa categories. A waiver is possible but requires a separate application and is not guaranteed.
What clinical documentation does a Graduate Nurse need before the DS-2019 is issued?
Before your designated sponsor can issue the DS-2019, you typically need a CGFNS credential evaluation confirming your nursing degree equivalency, evidence of English proficiency such as IELTS or TOEFL scores, a signed training plan detailing your clinical rotations, a letter of support from the host facility, and proof of adequate health insurance. Some sponsor organizations also require NCLEX passage or an active U.S. state nursing license depending on the training program's scope.
