KenCrest is seeking an exceptional healthcare leader to oversee two highly specialized residential healthcare programs serving medically fragile children and adults with complex medical needs. This position provides a unique opportunity to shape innovative services that transform lives, from helping children achieve developmental milestones in a transitional care setting to supporting adults with intellectual and developmental disabilities in living healthy, meaningful, and connected lives within their communities.
As the Director of Medical Homes, you will provide strategic and operational leadership for KenCrest's Children's Transitional Care Homes, Community Medical Homes, and Home Health Agency. You will oversee programs that blend advanced clinical care with a deeply person-centered philosophy, ensuring high-quality services across residential and community-based settings. This role is responsible for strengthening clinical and operational performance, ensuring regulatory excellence, and fostering environments where individuals receive outstanding care in places they proudly call home.

Key Responsibilities
Provide strategic leadership and operational oversight for KenCrest's Children's Transitional Care Homes, Community Medical Homes and Home Health Agency.
Oversee home health services to ensure the delivery of high-quality, coordinated care that supports individuals living safely and successfully in their homes and communities.
Ensure the delivery of high-quality, person-centered services that promote health, safety, dignity, independence, and quality of life.
Ensure compliance with all federal, state, accreditation, licensing, and regulatory requirements.
Lead preparation for and response to audits, surveys, inspections, and regulatory reviews.
Lead multidisciplinary teams supporting medically fragile children and adults with complex medical, developmental, and behavioral healthcare needs.
Partner with nursing and clinical leadership to advance clinical excellence, evidence-based practice, and positive health outcomes.
Oversee program performance, quality metrics, and continuous improvement initiatives across multiple locations.
Develop, monitor, and manage operational budgets while ensuring efficient and effective use of resources.
Drive workforce planning initiatives, including recruitment, retention, staff development, succession planning, and staffing optimization.
Ensure staffing models appropriately support census, clinical acuity, operational demands, and regulatory requirements.
Oversee the development, implementation, and evaluation of clinical protocols, operational procedures, and best-practice standards.
Foster a culture of accountability, collaboration, innovation, professional growth, and employee engagement.
Build and maintain strong partnerships with families, healthcare providers, regulatory agencies, community organizations, and other key stakeholders.
Utilize data, outcomes, and performance indicators to evaluate program effectiveness and drive ongoing improvements.
Serve as a visible ambassador for the programs and KenCrest's mission, both internally and externally.
Qualifications
Current Pennsylvania Registered Nurse (RN) license required.
Degree in Nursing, Healthcare Administration, or a related field required
Minimum of five years of progressive leadership experience in pediatric healthcare, residential healthcare, complex care, home health agency or related healthcare settings.
Demonstrated experience serving medically fragile children, adults with intellectual and developmental disabilities, or other medically complex populations.
Experience supporting individuals requiring ventilator care, tracheostomy management, respiratory support, and other advanced clinical interventions strongly preferred.
Strong knowledge of healthcare regulations, accreditation standards, risk management practices, quality improvement methodologies, and compliance requirements.
Proven success leading clinical, operational, financial, and workforce initiatives across multiple sites or programs.
Exceptional leadership, communication, relationship-building, and problem-solving skills.
Valid driver's license and ability to travel between program locations required
