Total Care Physicians is seeking a full time Nurse Practitioner (NP) or Physician's Assistant (PA) to join our Primary Care practice, delivering excellent patient care located in Delaware. NP/PA will provide preventive and sick care in an office setting, maintain accurate clinical records in our EHR system. Responsibilities:
- Provide office based clinical primary care including screenings, history and physicals and care plan documentation.
- Enhance delivery of well-coordinated care by incorporating records from acute, specialist and post-acute visits into the clinical chart and deriving clinical insights.
- Provide efficient care to minimize patient wait time and achieve practice productivity goals.
- Maximize patient satisfaction by providing evidence-based patient education, shared decision making and excellent bedside manner.
- Perform medical procedures as needed.
- Educate patients and families about health conditions, treatment plans and preventative care strategies.
- Collaborate with health care providers to develop and implement individualized care plans.
- Stay current with advancements in medical knowledge.
Qualifications:
- Current Nurse Practitioner or Physician's Assistant licensure in the state of Delaware.
- Experience preferred but not necessary in a primary care or comparable care setting.
- Strong skills in medical documentation and familiarity with electronic health care system (Athena).
- Strong written and interpersonal communication skills.
- Team oriented and a positive attitude.

Pay: $100,000.00 - $130,000.00 per year
