Job Description

This role offers clinical autonomy, flexible scheduling, and strong organizational support in post-acute and long-term care settings (SNF/LTC/ALF/IRF).
Role Overview
- Full-time, Monday-Friday schedule
- No nights or weekends
- Flexible, onsite, round-and-go model with remote documentation
- Conduct psychiatric evaluations, diagnostic assessments, and mental status examinations
- Diagnose and treat psychiatric, behavioral, and neurocognitive disorders in older adults
- Prescribe and manage psychotropic medications
- Evaluate behavioral and safety concerns
- Provide brief interventions and behavioral management
- Collaborate with interdisciplinary care teams across care settings
Compensation and Benefits
- Estimated compensation range of $150,000 – $200,000 annually with no cap on productivity income
- Base salary plus productivity, W2 position with full benefits (medical, dental, vision, PTO, 401k)
- Back-end infrastructure and clinical support provided
Qualifications
- Active, unrestricted New York license as a PMHNP-C or Psych PA-C
- Active DEA or willingness to obtain
- Open to new grad of experienced PMHNPs or Psych PAs
- Experience with older adults or post-acute care preferred
