Family Nurse Practitioner (NP) - Virtual Chronic Care | Boston, Massachusetts

ProMedix Health is a Medicare-focused virtual care company delivering Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs to patients with chronic conditions. We are seeking a licensed Family Nurse Practitioner based in the San Francisco Bay Area to join our growing clinical team.
About the Role
THIS POSITION ALLOWS FOR REMOTE WORK AND FLEXIBLE HOURS, HOWEVER;
PROOF OF RESIDENCY IN THE CITY OF BOSTON IS REQUIRED - NO EXCEPTIONS
This is a 1099 independent contractor position in which you will serve as the primary point of contact for new patient enrollment, building and maintaining patient-physician relationships from the ground up. Once enrolled, you will provide clinical oversight for patients within our RPM and CCM programs, working collaboratively with care coordinators and clinical staff operating under your general supervision. Clinical staff handle routine monthly CCM follow-up visits; the FNP is expected to be available to review escalations and intervene when clinically necessary.
Key Responsibilities
- Provide initial patient visits and enroll new patients into RPM and CCM programs and establish ongoing patient-provider relationships
- Provide clinical oversight and supervision for care coordinators and clinical staff delivering day-to-day monitoring and monthly CCM follow-up services
- Review escalated patient-generated health data (blood pressure, glucose, weight, etc.) and adjust care plans as needed
- Collaborate with physicians, specialists and the broader care team on escalations, referrals, and care transitions
- Ensure documentation and billing compliance with CMS guidelines for RPM and CCM
- Educate patients on chronic disease management, medications, and device adherence
Requirements
- Active, unrestricted NP license in Washington state, in good standing
- Underlying active, Compact RN license in Massachusetts, in good standing
- Current national board certification (AANP or ANCC)
- Minimum 3-5 years of NP experience in primary care, internal medicine, or family medicine
- Experience managing chronic conditions including hypertension, diabetes, and cardiovascular disease
- Active Medicare enrollment (or ability to obtain); no OIG/GSA exclusions
- Prior RPM, CCM, or telehealth experience strongly preferred
- Comfortable with EHR systems and virtual care platforms
- Must reside in the city of Seattle
Compensation
Competitive hourly rate of $65-$85/hr, commensurate with experience and credentials.
*The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials, and licensure. The range is contemplated to be between $65 - $85 per hour.
