Your Role: As a Hospice RN, you'll provide compassionate, patient-centered care while coordinating with an interdisciplinary team to ensure patients' physical, emotional, and spiritual needs are met.
What You'll Do:
- Plan, coordinate, and deliver hospice care according to individualized patient care plans.
- Collaborate with patients, families, and the interdisciplinary team (IDG) to ensure seamless care delivery.
- Conduct comprehensive assessments and develop care plans tailored to each patient's needs.
- Provide education and guidance to patients and families about the dying process and healthcare decisions.
- Perform and document patient evaluations, visits, and care in compliance with federal, state, and accreditation standards.
- Monitor patient conditions, report changes to the attending physician, and adjust care plans accordingly.
- Supervise and evaluate care provided by Hospice Aides and LPNs, ensuring high-quality service delivery.
- Maintain an up-to-date understanding of hospice regulations, current drug therapies, and best practices.
- Offer dietary counseling, addressing changes in diet at the end of life.
- Participate in IDG meetings, QAPI activities, and staff development opportunities.
Qualifications:
- Graduate of an accredited School of Nursing.
- Current RN license in IL.
- Minimum of 1 year of RN experience; hospice experience preferred.
- Exceptional assessment and problem-solving skills.
- Strong organizational and time-management abilities.
- Effective verbal and written communication skills.
- Licensed driver with a reliable, insured vehicle.
- Commitment to maintaining patient dignity, safety, and comfort.
