First Choice Health Centers is seeking a Nurse Practitioner or Physician Assistant for our family care practice in Manchester, CT. This position is accountable for independently performing a full range of tasks in delivery of direct patient care in an outpatient setting.

EXPERIENCE AND TRAINING:
- Proven experience as a Nurse Practitioner or Physician Assistant evaluating patients of all ages and establishing care plans
- A current license to practice as a NP or PA in the state of Connecticut
- Appropriate Board Certification
- A commitment to serving diverse and vulnerable populations with compassion and respect
- Excellent communication, interpersonal, and organizational skills
- Connecticut Controlled Substance license and DEA license
- BLS Certification through the American Heart Association or American Red Cross
- Pay is based on several factors including but not limited to work experience certifications, etc. Range for this position is $115,000 - $160,000
STANDARD JOB DUTIES:
- Evaluates, assesses patient illness/injuries assessing all ages
- Chronic care management
- Performs physical examinations, and special examinations
- Takes medical histories
- Develops plans of care
- Provides routine and illness care
- Orders medications; orders diagnostic tests; and evaluates results
- Assumes responsibility for patient follow-up, coordination, and referrals to other health care providers as necessary
- Maintains patient medical records
- Provides patient counseling/education
- Participates in medical student and health professional teaching
- Performs related duties as required
