OPT Neonatologist Jobs
Neonatologist jobs on OPT require a completed U.S. medical residency and fellowship, making this one of the longer training-to-practice paths for F-1 students. Most positions are hospital-based, and many health systems have experience sponsoring H-1B visas, which makes the transition from OPT STEM or standard OPT more straightforward than in other specialties.

Get Access To All JobsTips for Finding OPT Sponsorship in Neonatologist
Confirm your OPT authorization covers clinical work
Neonatology roles involve direct patient care, which counts as practical training under OPT. Ensure your EAD is active and your employer is properly listed with your DSO before you begin seeing patients independently or supervising residents.
Target health systems with established J-1 and H-1B pipelines
Large academic medical centers and children's hospitals regularly sponsor international physicians. These institutions have immigration counsel on staff and understand the visa process, making them far more likely to sponsor your H-1B after OPT ends.
Start H-1B conversations during your fellowship, not after
OPT has a fixed endpoint. Raising sponsorship with your program director or department chair 12 to 18 months before your authorization expires gives HR and legal counsel enough time to prepare the petition without rushed timelines.
Understand how your training timeline intersects with OPT
Neonatology fellowship typically runs two to three years. If your OPT begins mid-fellowship, track expiration carefully. Some fellows bridge on cap-exempt H-1B through their teaching hospital before transitioning to a standard H-1B.
Get board certification on track before OPT expires
Neonatologist positions almost universally require or strongly prefer board certification in neonatal-perinatal medicine. Employers are more willing to sponsor candidates who are board-eligible or certified, as it reduces hiring risk during the visa process.
Use Migrate Mate to find OPT-friendly neonatology openings
Neonatologist roles that explicitly welcome OPT applicants are not always easy to find. Browse Migrate Mate to filter positions by visa sponsorship willingness, saving time you would otherwise spend screening roles that exclude international candidates upfront.
Neonatologist OPT: Frequently Asked Questions
Can I work as a neonatologist on OPT without an H-1B?
Yes, if your EAD is active and your position is directly related to your field of study, you can work as a neonatologist during your OPT period. However, neonatology is a long-training specialty, so most F-1 students reach this stage after residency and fellowship, leaving limited OPT time. Coordinating your OPT start date carefully with your program and DSO is essential.
Does neonatology qualify for STEM OPT extension?
Neonatology itself is a medical specialty, and whether it qualifies for STEM OPT depends on the CIP code associated with your degree. Medical degrees classified under certain biological science or biomedical engineering codes may qualify, but many MD programs do not fall on the STEM OPT eligible list. Check with your DSO to confirm your specific degree's eligibility before assuming a 24-month extension is available.
How do I find neonatologist jobs that sponsor OPT and H-1B?
The most efficient approach is to focus your search on positions that explicitly indicate openness to visa sponsorship. Migrate Mate is built specifically for this, allowing you to browse neonatologist openings filtered by sponsorship availability. Academic children's hospitals and large health systems are also worth targeting directly, as they sponsor international physicians regularly and often have streamlined processes for H-1B petitions after OPT.
What happens to my OPT status if my neonatology fellowship changes programs?
Changing fellowship programs constitutes a change of employer under OPT rules. You must report the change to your DSO within 10 days and update your SEVIS record. If you have a STEM OPT extension, your new employer must also submit a new Form I-983 training plan. Failing to report the change can jeopardize your OPT authorization, so act immediately when any program transition occurs.
Can a neonatologist on OPT work at multiple hospitals or as an independent contractor?
OPT permits multiple concurrent employers as long as each position is directly related to your field of study and you work at least 20 hours per week in aggregate. However, independent contractor arrangements require careful documentation, particularly under STEM OPT, where each employer must sign a training plan. Self-employment is generally not permitted under standard OPT. Consult your DSO before accepting per diem or locum tenens arrangements.
