- Seeking experienced Nurse Practitioner candidates that have an active, unrestricted Family Or Adult Nurse Practitioner license in the state of Virginia AND Must Have a Current, Active RN Compact license.
LOCATION
Location: Virtual - This role enables associates to work virtually full-time, except for required in-person training sessions, providing maximum flexibility and autonomy. This approach promotes productivity, supports work-life integration, and ensures essential face-to-face onboarding and skill development. Alternate locations may be considered if candidates reside within a commuting distance from an office.

WORK SHIFT
Work Shift: Monday – Friday, 8:00 am to 5:00 pm (EST or CST) And rotating on-call.
ROLE AND RESPONSIBILITIES
The Advance Practice Provider, Nurse Practitioner is responsible for collaborating with company physicians, the patient’s other physicians and providers, and their family members to develop complex plans of care in accordance with the patient’s health status and overall goals and values. Provides clinical and non-clinical support to patients.
How you will make an impact:
- Provides primary care and acute care via telephone and tele video modalities to patients who receive home and community-based services through state Medicaid programs, dual eligible members and other membership as assigned by our MCO partners.
- Develops and implements clinical plans of care for adult patients facing chronic and complex conditions (e.g., co-morbid medical and mental health diagnoses, limited personal resources, chronic medical conditions).
- Gathers history and physical exam and diagnostics as needed, and then develops and implements treatment plans given the patient’s goals of care and current conditions.
- Identifies and closes gaps in care.
- Meets the patient’s and family’s physical and psychosocial needs with support and input from the company’s inter-disciplinary team.
- Educates patients and families about medication usage, side effects, illness progression, diet and nutrition, medical adherence and crisis anticipation and prevention.
- Maintains contact with other clinical team members, patients’ other physicians and patients’ other medical providers to coordinate optimal care and resources for the patient and his or her family in a timely basis and consistent with state regulations and company health standards and policy.
- Maintains patient medical records and medical documentation consistent with state regulations and company standards and policy.
- Participates in continuing education as required by state and certifying body.
- Prescribes medication as permitted by state prescribing authority.
MINIMUM QUALIFICATIONS
- Requires a MS in Nursing.
- Requires an active, national NP certification.
- Requires valid, current, active and unrestricted Family or Adult Nurse Practitioner (NP) license in the state of Virginia.
- Valid, current, active, RN Compact license is required.
- Requires 2+ years of experience in managing complex care cases.
- Experience working with Electronic Medical Records (EMR) required.
PREFERRED QUALIFICATIONS
- Possession of DEA registration or eligibility preferred.
- Active Medicaid number in the state of Virginia highly preferred.
- Experience in managing complex care cases for developmental disabilities and chronically ill patients strongly preferred.

Most states require a separate NP license beyond your RN license. Licensing timelines vary by state and can take several months. Delays in licensure can make you ineligible to work in clinical roles, burning through your OPT window.
Address cap-gap proactively if your OPT expires before October
If your OPT end date falls before October 1 and your employer files an H-1B petition on your behalf, cap-gap protection automatically extends your work authorization. Confirm your employer understands this before your OPT expires.
Primary Care Nurse Practitioner OPT: Frequently Asked Questions
Can I work as a Primary Care Nurse Practitioner on F-1 OPT?
Yes, as long as your NP employment is directly related to your degree program, typically an MSN, DNP, or post-graduate NP certificate. You must have a valid EAD card before starting work. Because most NP programs are not STEM-designated, you are limited to 12 months of post-completion OPT unless your specific program qualifies for an extension.
Do Primary Care NP jobs typically lead to H-1B sponsorship?
Many do, particularly at large health systems, federally qualified health centers, and hospital-affiliated primary care networks. NP roles qualify as specialty occupations under H-1B because they require at minimum a master's degree in nursing. Does my NP license affect my OPT eligibility or work start date?
Your OPT authorization and your NP license are separate requirements. USCIS approves your EAD based on your degree program, not your licensure status. Apply for licensure as early as your state allows.
Is a DNP considered a STEM degree for the 24-month OPT extension?
No. Doctor of Nursing Practice programs are classified under the CIP code for nursing, which is not included on the DHS STEM Designated Degree Program List. Most MSN and NP certificate programs are also excluded. This means the standard OPT period for most Primary Care NP graduates is 12 months. Verify your program's CIP code with your DSO if you are unsure.
What employment settings count as authorized OPT work for an NP?
Any setting where you practice in your authorized NP scope qualifies, including physician offices, community health clinics, urgent care centers, and telehealth platforms operating under a supervising physician or collaborative practice agreement. Volunteering or unpaid positions can count toward OPT requirements only if they meet the 20-hours-per-week minimum and are directly tied to your NP field of study. Confirm each arrangement with your DSO before accepting.
