Get Access To All JobsTips for Finding OPT Sponsorship as a Primary Care Physician
Target federally qualified health centers
FQHCs and rural health clinics frequently sponsor physicians through H-1B visa and J-1 visa waiver programs. Their federal funding structures make them familiar with immigration processes, and many have dedicated HR staff who handle physician visa sponsorship routinely.
Understand your J-1 waiver options early
If you completed residency on a J-1 visa, you need a waiver before H-1B sponsorship is possible. Conrad 30 waivers through state health departments are the most common path for primary care physicians. Start this process before OPT ends.
Apply to Health Professional Shortage Areas
HPSAs and medically underserved areas actively recruit international physicians and are more experienced with sponsorship. Federal and state incentive programs tied to these designations give employers strong financial motivation to work through the visa process.
Confirm licensure timelines align with OPT
State medical board processing ranges from six weeks to several months. Apply for your medical license as early as possible so licensure delays don't create gaps between your OPT authorization and your employment start date at a sponsoring employer.
Negotiate sponsorship terms before signing
Ask explicitly whether the employer covers H-1B attorney fees and filing costs. Large hospital systems and multispecialty groups are more likely to absorb these costs than small independent practices, which may ask physicians to share or fully cover them.
Document your clinical training for petitions
H-1B petitions for physicians require detailed evidence of specialty qualifications. Compile your residency completion letter, ECFMG certificate, board examination results, and any fellowship documentation now so your employer's attorney can file without delays.
Primary Care Physician OPT: Frequently Asked Questions
Can I work as a Primary Care Physician on OPT before completing residency?
No. Primary care physician roles require completion of an accredited residency program and a valid state medical license. OPT authorizes employment in your field of study, but you cannot practice medicine independently without meeting licensure requirements in the state where you plan to work.
Which visa sponsorship pathway is most common for OPT physicians transitioning to long-term status?
H-1B sponsorship is the most common long-term pathway for primary care physicians. If you completed residency on a J-1 exchange visitor visa, you must first obtain a J-1 waiver, typically through the Conrad 30 program, before an employer can file an H-1B petition on your behalf.
Where can I find Primary Care Physician jobs with confirmed visa sponsorship?
Migrate Mate is built specifically for F-1 OPT students and filters for employers willing to sponsor international physicians. Searching on Migrate Mate saves time compared to applying broadly and discovering during the offer stage that an employer won't support visa sponsorship.
Does OPT STEM extension apply to Primary Care Physicians?
It depends on your degree. An MD or DO from a U.S. accredited medical school does not fall under a STEM-designated Classification of Instructional Programs (CIP) code, so most primary care physicians are not eligible for the 24-month STEM OPT extension. Confirm your program's CIP code with your DSO.
How does the 60-day grace period affect a Primary Care Physician's job search?
If your OPT employment ends before you secure a new sponsoring employer, you enter a 60-day grace period during which you cannot work. For physicians, credentialing and contract timelines at hospitals can extend two to four months, so beginning your job search well before OPT expires is important.
