How to Get Visa Sponsorship as a Registered Nurse Case Manager
Confirm your RN license before applying
Each state issues its own RN license, and most employers won't extend an offer without one. Apply for licensure through the state board early, processing can take six to twelve weeks, which can eat into your OPT window.
Verify your degree's STEM OPT eligibility
Some nursing programs fall under STEM-eligible CIP codes, giving you an additional 24 months of work authorization. Check with your DSO before your standard OPT expires so you can file the extension request on time.
Target health systems with established OPT hiring processes
Large hospital networks and integrated health systems hire internationally educated nurses more regularly than smaller clinics. They often have HR teams familiar with EAD verification, which reduces delays and confusion during onboarding.
Apply before your OPT start date if possible
Many case management roles require credentialing and onboarding paperwork that takes four to six weeks. Starting the application process early gives you time to complete these steps without losing authorized workdays on your EAD window.
Highlight case management certifications alongside your RN credentials
A CCM or ACM certification signals specialized competency and makes you more competitive against domestic applicants. Employers weighing the administrative effort of hiring an OPT worker are more likely to proceed when your qualifications are clearly distinguished.

Can F-1 OPT students work as Registered Nurse Case Managers in the United States?
Yes, F-1 OPT students can work as Registered Nurse Case Managers as long as they hold a valid EAD card and an active RN license in the state of employment. The role must be directly related to your nursing degree. You can browse OPT-eligible case management roles on Migrate Mate.
Do Registered Nurse Case Manager jobs qualify for the STEM OPT extension?
It depends on the CIP code assigned to your specific nursing program. Some nursing degrees, particularly those classified under health informatics or clinical informatics tracks, fall under STEM-eligible codes. Standard RN programs typically do not. Confirm your program's CIP code with your DSO before assuming you qualify for the 24-month extension.
Do I need a state RN license to apply for case manager roles on OPT?
Yes, a valid RN license in the state where you plan to work is a non-negotiable requirement for virtually all Registered Nurse Case Manager positions. Many states participate in the Nurse Licensure Compact, which allows a single license to cover multiple states. If you haven't applied yet, start the process early since approval timelines vary by state board.
Will employers sponsor Registered Nurse Case Managers for an H-1B visa after OPT?
Some do, particularly large hospital systems, managed care organizations, and insurance companies with established immigration programs. Case management roles generally qualify as specialty occupations under H-1B criteria because they require a bachelor's degree in nursing or a related clinical field. Raising the conversation during the offer stage gives you the clearest picture of each employer's willingness.
What employment settings hire Registered Nurse Case Managers on OPT?
The most common employers are hospital discharge planning departments, health insurance companies, accountable care organizations, and home health agencies. Telehealth platforms have also expanded case management hiring significantly. Settings that handle high patient volumes tend to have more structured HR processes for verifying EAD authorization, which can make onboarding smoother for OPT workers.
