What You'll Do
- Evaluate patients and develop individualized plans of care.
- Provide treatment for strength, balance, mobility, gait, and functional independence.
- Address fall prevention and home safety.
- Monitor patient progress and adjust treatment plans as needed.
- Collaborate with patients, families, and the interdisciplinary team.
- Complete timely and accurate clinical documentation.
Qualifications
- Active North Carolina Physical Therapist (PT) license.
- Strong clinical judgment, communication, and organizational skills.
- Dependable, compassionate, and comfortable working independently.
- Ability to manage a flexible schedule and travel to patients' homes in South Charlotte.
Home health, mobile therapy, or geriatric experience is a plus, but not required.
