Overview:

Island Health Care, a Federally Qualified Health Center (FQHC) on Martha's Vineyard, is seeking a compassionate and collaborative Family and Psychiatric Nurse Practitioner to join our integrated behavioral health team. This unique role supports our mission to expand access to high-quality, equitable care for underserved populations through a whole-person approach to health and wellness. In this dual-capacity position, you will provide psychiatric assessment, diagnosis, and medication management, while also delivering comprehensive primary care services including preventive care, chronic disease management, and acute care. You will manage a small panel of patients with complex medical and behavioral health needs, ensuring coordinated, patient-centered care across the continuum.

Working as part of a multidisciplinary team, you will collaborate closely with primary care providers, therapists, care coordinators, community health workers, and peer recovery coaches to integrate behavioral and physical health services and improve health outcomes. The ideal candidate is a skilled clinician, effective communicator, and committed advocate for health equity who is passionate about integrated care and dedicated to delivering trauma-informed, culturally responsive services.

Qualifications :

• 2+ years as a psychiatric nurse practitioner with experience with mental health assessment, therapeutic counseling, diagnosis, triage, managing common psychiatric medication and treatment plans, and managing crisis situations.
• Previous primary care experience in clinic or private practice settings strongly preferred.
• Masters of Science in Nursing with the successful completion of an approved nurse practitioner program.
• Current Massachusetts RN license, current NP Board certification, current Drug Enforcement Agency certificate, current Controlled Substance Registration, and current BLS certification.
• Experience with the FQHC setting.
• Knowledge of the principles and practices of community mental health and substance use disorder treatment.
• Excellent communication and interpersonal skills; team-oriented individual.
• Ability to work collaboratively with other members of the healthcare team.
• Strong leadership and organizational skills.
• Strong written and verbal communication skills.
• Ability to work with diverse populations, including individuals with limited English proficiency.

Essential Functions:

• Provide comprehensive, patient-centered primary care and behavioral health services across the lifespan, including assessment, diagnosis, treatment, health promotion, disease prevention, and ongoing care management.
• Conduct comprehensive medical and psychiatric evaluations, diagnose and manage acute and chronic physical and behavioral health conditions, order and interpret diagnostic testing, and develop, implement, and monitor individualized treatment plans.
• Prescribe, manage and monitor medications, including psychotropic medications, while evaluating treatment effectiveness, patient adherence, and potential side effects.
• Deliver brief therapeutic interventions, counseling, psychoeducation, and wellness education to support behavior change, chronic disease management, mental health, and overall wellbeing.
• Coordinate whole-person care for patients by collaborating with patients, families, interdisciplinary team members, specialists, and community partners, and by addressing social determinants of health through referrals and resource navigation.
• Support integrated care initiatives through consultation, clinical guidance, supervision, mentorship, participation in care team meetings, and collaboration across primary care and behavioral health disciplines.
• Maintain accurate and timely clinical documentation, ensure compliance with organizational, regulatory, accreditation, and quality standards, and participate in quality improvement and performance initiatives.
• Maintain professional competency through continuing education, professional development, and culturally responsive care that meets the needs of diverse patient populations.
• Attend provider meetings, case conferences, and interdisciplinary care team meetings to support coordinated care delivery and organizational initiatives.

Pay: $65.00 - $75.00 per hour
