What You'll Do
As a Physical Therapist, you'll evaluate, develop, and implement individualized treatment plans that help residents improve mobility, restore function, and maximize independence.
Your responsibilities will include:
Perform comprehensive physical therapy evaluations
Develop and implement individualized treatment plans based on resident goals
Provide skilled therapeutic interventions to improve strength, balance, mobility, and overall function
Collaborate with physicians, nursing staff, and the interdisciplinary care team to coordinate resident care
Educate residents and families on therapeutic exercises, mobility techniques, and fall prevention
Accurately document evaluations, treatments, progress notes, and discharge planning
Monitor resident progress and adjust treatment plans to maximize outcomes
Qualifications
Current California Physical Therapist (PT) license (or eligibility for licensure)
Strong clinical, communication, and problem-solving skills
Passion for rehabilitation and resident-centered care
Positive, collaborative, and compassionate approach to patient care
