With a network of over 150 providers across Florida, Illinois, Indiana, Michigan, Missouri, Kansas, Texas, Oregon, Utah, and Washington State, we deliver comprehensive care to residents in skilled nursing, long-term care, and assisted living facilities. We are currently seeking a Pulmonology Medical Director based in St. Louis, Missouri, to oversee our Advanced Practice Clinicians (APCs) and serve as a clinical ambassador for our facilities. This is a part-time position.

Duties and Responsibilities:
Facility Oversight: Conduct facility visits at least once per month or as required.
Clinical and Educational Leadership:
- Meet monthly with each supervised Advanced Practice Clinician.
- Review and authorize five clinical notes per week for each supervised APC.
- Lead educational presentations at skilled nursing facilities to train nursing staff on cardiac-related topics.
- Deliver professional development presentations to CardioRenal Vision Advanced Practice Clinicians.
- Develop and implement best-practice care models and provide comprehensive training to supervised staff.
- Interpret diagnostic tests performed by Advanced Practice Clinicians.
- Facilitate communication between families and healthcare professionals involved in patient care.
Marketing and Outreach:
- Participate in in-person and virtual meetings with potential facilities to present our clinical services.
- Establish and maintain relationships with local hospitals and health systems to improve continuity of care and care transitions.
