Job Title: Registered Nurse
Department: Home Care
Location: ElderOne -Seneca Towers, 200 Seth Green Drive Rochester, NY 14621
Hours Per Week: 40 hours
Schedule: Monday - Friday 8am -4:30-pm, 1 weekend per quarter.
RESPONSIBILITIES
Patient Care. Conduct initial assessments; establish plan of care and provide prescribed treatments and demonstrated nursing abilities; coordinate the delivery of 24-hour care;
Communication. Counsel and guide individuals and families towards self-help in recognition and solution of physical, emotional and environmental problems
Evaluation & Documentation. Conduct reassessment of each patient on a regular basis; maintain complete and accurate clinical documentation; supervise and serve as a resource to all home care staff
REQUIRED QUALIFICATION: Nursing Diploma or ASN, Registered RN in State of NY, NY driver's License
PREFERRED QUALIFICATION:
BSN
1 year of experience working with a frail or elderly population preferred.
Availability of Motor Vehicle
6 months- 1 year experience conducting UAS-NY Community Assessment preferred.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
