Good Shepherd Program, Supervisor

SUMMARY:
Provide case management/social work services to clients of Good Shepherd Residential Program, according to social service standards of Catholic Social Services, Saint John’s Hospice, Mission Statement and the requirements of contracting agencies.
PRINCIPLE DUTIES AND RESPONSIBILITIES:
- Complete HUD Entry and Exit assessments and intake within required timeframes.
- Conduct comprehensive assessments (HUD, VI-SPDAT) and verify eligibility.
- Track referrals in Clarity HMIS, Apricot and conduct intake assessment within 24 hours.
- Develop housing-focused Individual Service Plans with measurable goals within 10 days of intake.
- Meet biweekly with participants to monitor progress and adjust housing plans as needed.
- Explore housing options and assist participants with applications and documentation.
- Support participants in developing income stability, budgeting, and tenant skills.
- Connect participants to community resources including healthcare, employment, education, and financial literacy.
- Assist with scheduling and attending medical, behavioral health, and benefits appointments.
- Manage medication storage, documentation, and disposal in compliance with agency policy.
- Conduct Participant Progress Reviews for clients facing barriers or transitioning from services.
- Maintain accurate and timely documentation in all required databases (Apricot, CareWare, Clarity HMIS).
- Record case notes in D-A-P format at least twice monthly.
- Complete exit interviews and ensure all close-out documentation is submitted.
- Maintain organized and confidential client case files in accordance with agency standards.
- Deliver on-site case management services that align with OHS Emergency Housing Standards.
- Attend biweekly supervision sessions and participate in ongoing professional development.
- Maintain current CPR certification and crisis management training.
- Participate in resident meetings and internal or external case conferences as assigned.
- Perform other duties as assigned to support program operations and client outcomes.
KNOWLEDGE, SKILLS AND ABILITIES:
- Bachelor’s degree in social work (BSW) or related field.
- Two years of social work or case management experience.
- Skilled in assessment, service planning, and documentation.
- Proficient in case management software and data entry systems (HMIS preferred).
- Knowledge of homelessness, housing first, trauma-informed care, and behavioral health.
- Strong communication, organization, and problem-solving skills.
- Ability to build rapport while maintaining professional boundaries.
- Experience working with diverse populations and individuals living with HIV/AIDS.
- Knowledge of community resources and referral processes.
Education:
BSW (Preferred) or comparable bachelor’s degree
Experience:
Two years’ social work / case management experience
Physical Requirements:
Ability to stand, sit, walk, climb stairs, operate controls, reach out with hands, occasionally lift objects up 40 lbs.
