INTRODUCTION
Cleveland Clinic is seeking a Board Certified / Board Eligible Internal Medicine Hospitalist to join our rapidly growing post-acute care program within the Long Term Acute Care Hospital (LTACH) environment. This innovative role supports our mission to deliver world class care across the full continuum, ensuring safe, high-quality transitions from acute hospitalization back to the community.

Practice Environment
Providers will be assigned to one or more community based LTACH facilities, caring for medically complex patients requiring extended recovery and interdisciplinary management. You will work closely with:
- Cleveland Clinic consulting specialists
- Nursing, pharmacy, therapy, and care management teams
- Post-acute and community partners to ensure continuity of care
BASIC QUALIFICATIONS
- MD or DO
- Board Certified or Board Eligible in Internal Medicine
- Interest in post-acute medicine, care transitions, and multidisciplinary care
- Commitment to patient-centered, high-quality clinical practice
