INTRODUCTION
The Psychosocial Rehabilitation (PSR) Specialist provides structured, goal-oriented rehabilitative services to individuals with behavioral health conditions. Services are person-centered and recovery-oriented, focusing on strengthening skills necessary for independent living, social integration, and community participation.
The PSR Specialist collaborates with an interdisciplinary team to implement services outlined in the Individualized Recovery Plan (IRP) and supports individuals in achieving measurable outcomes aligned with treatment goals.
MINIMUM QUALIFICATIONS
Education:
- Bachelor’s degree in a human services field (e.g., psychology, social work, rehabilitation counseling, or related discipline) required
Experience:
- At least one (1) year of experience in behavioral health, rehabilitation, or related human services preferred
Required Skills & Competencies
- Knowledge of psychiatric rehabilitation and recovery-oriented practices
- Understanding of trauma-informed care
- Strong interpersonal and communication skills
- Ability to manage multiple priorities and meet documentation deadlines
- Proficiency in Microsoft Office, Google Workspace, and EHR systems
- Ability to work both independently and as part of a multidisciplinary team

ESSENTIAL JOB FUNCTIONS
- Provide rehabilitative, skills-based interventions tied to functional deficits identified in treatment plans
- Deliver individual and group PSR services to support:
- Activities of daily living
- Social and interpersonal skills
- Cognitive and coping skills
- Community integration
- Use evidence-informed techniques such as modeling, coaching, and role-playing
- Support individuals in achieving measurable, goal-oriented outcomes
INDIVIDUALIZED RECOVERY PLANNING
- Assist in development and implementation of treatment plans
- Ensure all services are linked to documented goals and medical necessity
- Monitor and document progress; adjust interventions as needed
SKILL BUILDING & COACHING
- Provide hands-on instruction in real-life and simulated environments
- Support development of communication, problem-solving, and self-management skills
- Assist with daily living tasks such as budgeting, scheduling, and accessing services
TEAM COLLABORATION
- Participate in interdisciplinary team meetings and case conferences
- Communicate regularly with team members regarding progress and service needs
COMMUNITY-BASED SUPPORT
- Connect individuals to community resources (housing, employment, healthcare, etc.)
- Provide services in community and home settings
- Support transition and discharge planning
DOCUMENTATION & COMPLIANCE
- Maintain accurate, timely documentation in the electronic health record (EHR)
- Ensure compliance with Medicaid, New York State, and agency requirements
- Document services, interventions, and outcomes in accordance with billing standards
WORK ENVIRONMENT
Services are delivered in clinics, community locations, and individuals’ homes. The position requires travel within the service area and active engagement in community-based settings.
