Transition Of Care Jobs in California: Frequently Asked Questions
How do you become a transition of care in California?
Most transition of care roles in California require an active registered nurse license issued or recognized by the California Board of Registered Nursing, earned after completing an accredited nursing program and passing the NCLEX-RN. Some positions are open to licensed clinical social workers credentialed through the California Board of Behavioral Sciences. Employers in California typically require additional experience in case management, discharge planning, or utilization review before hiring into a dedicated transition of care role.
Which companies hire transition of cares in California?
California transition of care roles are posted by Stanford Health Care, Clever Care Health Plan, and Health Net and others right now, based on current listings on Migrate Mate as of October 2026. California's large integrated health systems and managed care plans generate consistent demand because of the state's high patient volume and complex post-acute care landscape.
Which California cities have the most transition of care jobs?
Palo Alto, California, and Arcadia have the most transition of care openings in California. Los Angeles and San Francisco drive the highest volume because of the concentration of large academic medical centers and managed care headquarters located there, while San Diego's openings reflect the strong presence of military-affiliated and regional health systems that run active care coordination programs.
Are there remote transition of care jobs in California?
Yes, but they are less common than in purely desk-based fields because many transition of care responsibilities involve direct patient contact, in-person assessments, and coordination with bedside teams. About 100% of transition of care openings tied to California are remote or hybrid as of October 2026, and those positions tend to focus on telephonic case management, utilization review, or post-discharge follow-up calls rather than bedside coordination.
How can I get hired as a transition of care in California with little or no experience?
The most realistic entry path is securing an RN position on a medical-surgical or progressive care unit at a large California health system, then moving laterally into a discharge planning or care coordination role once you have inpatient experience. Kaiser Permanente, Dignity Health, and Sutter Health all run new-graduate nurse residency programs in California that build the clinical foundation employers expect. Earning a case management certification early and gaining familiarity with Medi-Cal requirements significantly strengthens a candidate's transition into the role.
Where can I find and apply to transition of care jobs in California?
