Role Description
The Advanced Practice Provider (APP) – Primary Care will deliver comprehensive, evidence-based primary care services to patients across the lifespan in an outpatient setting. This full-time, on-site role based in CT (Naugatuck Valley) includes conducting patient assessments, performing physical exams, ordering and interpreting diagnostic tests, and developing individualized treatment plans in collaboration with supervising physicians as appropriate. The APP will manage chronic and acute conditions, provide preventive care and health education, coordinate referrals to specialty services, and ensure continuity of care. Daily responsibilities include documenting encounters in the electronic health record, participating in multidisciplinary team meetings, adhering to clinical protocols and regulatory standards, and contributing to quality improvement initiatives that support Griffin Hospital’s patient-centered care mission.
Qualifications
- Demonstrated clinical proficiency in Medicine and Primary Care, including management of common acute and chronic conditions.
- Experience or training in Critical Care Medicine and Emergency Medicine sufficient to recognize and appropriately respond to urgent or complex patient presentations.
- Current licensure and certification as an Advanced Practice Provider (such as Nurse Practitioner or Physician Assistant) in the state of Connecticut, with eligibility for prescriptive authority.
- Strong patient-centered communication skills, including the ability to educate patients and families, collaborate with interprofessional teams, and document clearly and accurately.
- Graduate degree from an accredited APP program; board certification in a relevant specialty (e.g., Family or Adult-Gerontology) preferred.
- Commitment to evidence-based practice, continuous learning, and participation in quality and safety initiatives.
- Ability to work independently within established protocols, exercise sound clinical judgment, and manage a diverse primary care panel.
- Prior experience in community or hospital-based primary care settings is highly desirable.
