Responsibilities
- Complete initial and ongoing physical therapy evaluations in patients’ homes
- Develop and carry out individualized treatment plans based on physician orders and patient goals
- Provide skilled interventions including gait training, strengthening, balance training, transfer training, mobility training, and fall prevention
- Assess home safety and recommend equipment or modifications as needed
- Educate patients, families, and caregivers on exercises, precautions, and safety strategies
- Collaborate with physicians, nurses, and interdisciplinary team members to support continuity of care
- Complete accurate and timely documentation in compliance with company and regulatory standards
- Monitor patient progress and adjust treatment plans as appropriate
Qualifications
- Current, active Physical Therapist (PT) license in [State]
- Graduate of an accredited Physical Therapy program
- Minimum of 1 year of clinical experience preferred
- Home Health experience preferred
- Current CPR certification
- Valid driver’s license, reliable transportation, and current auto insurance
