Description Of Position
The Clinical Case Management Social Worker performs Social Service assessment and addresses counseling needs.
SUPERVISION: Reports directly to Director of Clinical Case Management
Responsibilities And Duties
- Develops and maintains thorough knowledge of referral resources throughout the service area.
- Facilitates home health and nursing home placements and transfers.
- Provides positive customer relations in dealing with patients, families, physicians, fellow department employees, other hospital departments, referral resources, service agencies and others.
- Practices proactive discharge planning and coordination of care.
- Maintains accurate and timely records, including clinical and progress notes, according to department policy.

Education And Work Experience
- Master's in Social Work, Case Management certification preferred.
- One (1) year of social work experience in a health care setting.
- Computer skills at data entry level.
- Must maintain required hours of in-service per year to meet requirements for CEU's as stipulated by the Louisiana State Board of Social Work Examiners (20 hours).
- Ability to communicate with patient, family peers and medical personnel and community referral agencies.
Physical Demands/Work Environment
- Work requires a variety of physical activities, including moving about within and outside of all hospital properties for long periods of time.
- Must be able to respond quickly and effectively to emergency and non-emergent situations.
- May be required to assist in controlling disorderly conduct or combative patients.
- Must be able to exchange accurate information with patient, family, peers and medical personnel.
