Responsibilities:
The Social Worker is a member of the multidisciplinary team. The Social Worker collaborates directly with the care team, care management team and patient, and/or family, and/or the patient’s representative to assess the patient’s continuum of care needs and ensure the patients transition to the least restrictive care environment. As an independent professional, the Social Worker assesses the patient’s psychosocial and continuum of care needs. Interventions may include, but are not limited to, family support, crisis management, consultations with a staff physicians, Adult or Child Protective Services reporting/coordination, custody/guardianship issue coordination, indigent patient program coordination, substance-use support coordination, intimate partner violence support, bereavement. Additionally, interventions may include referrals and coordination of acute rehabilitation, extended care, durable medical equipment, home health, and other resources as necessary. The Social Worker adheres to all Norton Healthcare policies and applies his/her educational and professional expertise in the delivery of quality care.
QUALIFICATIONS
Required:
* One year social work or One year social work internship. Must have hospital and/or payer credentialing completed before functioning in this role.
* Master Degree in Social Work from accredited program
* Kentucky - Certified Social Worker (CSW), or Indiana - Licensed Social Worker (LSW)

* Licensed Clinical Social Worker (LCSW)
