- Develop individualized treatment plans to improve strength, flexibility, balance, and coordination.
- Provide therapeutic interventions including mobility training, strength exercises, manual therapy, and balance rehabilitation.
- Assist residents recovering from surgery, illness, stroke, or chronic conditions.
- Monitor therapy progress and update treatment programs based on patient performance and goals.
- Educate residents and caregivers on safe mobility practices, fall prevention, and home exercise programs.
- Maintain accurate therapy documentation including evaluations, treatment notes, and discharge summaries.
- Work collaboratively with interdisciplinary care teams including nursing staff, physicians, occupational therapists, and speech therapists.
- Participate in care planning meetings and support safe discharge planning.
Requirements
- Active Physical Therapist License
- Degree from accredited Physical Therapy program
