INTRODUCTION
St. Mary's Medical Center is seeking a Full Time Medical Social Worker - MSW for our Care Management Department. The Medical Social Worker (MSW) exercises independent judgement and professional discretion in the practice of case management to a selected group of patients. The MSW collaborates with attending physicians and residents and serves as advocate to help patients and families navigate the acute episode of care. The MSW monitors patient's continuing stay for resource utilization and helps prepare them for a safe and timely discharge. The MSW is proactively uses collaborative problem solving skills to meet the needs of the patients and physician. The MSW provides a full range of medical social work services as needed and relies heavily on such resources as family, community, state and federal agencies as well as the patient's resources to assist the patient's return in a productive life.
BASIC QUALIFICATIONS
Education Requirements: Minimum of Bachelors Degree (BSW) from approved school of Social Work, holds a current license in the state of West Virginia. No temporary permits accepted.
Certifications/Skills: Certifications in Care Management preferred, not required. BLS required.
Experience: MSW Social Worker, Licensed (MSW) Social Worker should have one (1) year of clinical Social Work experience, community based and/or hospital based.
