How to Get Visa Sponsorship in PT
Target healthcare systems and rehabilitation hospitals
Large hospital networks like HCA Healthcare, Kaiser Permanente, and Ascension regularly sponsor H-1B visas for physical therapists due to ongoing staffing needs and established immigration processes.
Highlight your DPT degree and clinical specializations
Physical therapy requires specific education credentials. Emphasize your Doctor of Physical Therapy degree, clinical rotations, and any specializations like orthopedics, neurology, or pediatrics when applying.
Obtain state licensure before or during the sponsorship process
Most states require PT licensure to practice. Some employers will sponsor while you complete licensure requirements, but having it demonstrates commitment and removes a potential hiring barrier.
Consider underserved rural areas with visa incentives
Rural hospitals and clinics often face PT shortages and may offer visa sponsorship, relocation assistance, and competitive packages to attract international candidates to underserved communities.
Research employers with established J-1 to H-1B pathways
Some healthcare facilities hire international PTs on J-1 visas first, then transition to H-1B sponsorship. This creates a clearer path from temporary to permanent work authorization.
Demonstrate specialized skills in high-demand areas
Physical therapists with expertise in areas like vestibular rehabilitation, wound care, or lymphatic drainage often have stronger sponsorship prospects due to specialized knowledge requirements and patient demand.

Get Access To All JobsFrequently Asked Questions
Do physical therapists qualify for H-1B visa sponsorship?
Yes, physical therapy is considered a specialty occupation under H-1B requirements. The role requires a Doctor of Physical Therapy (DPT) degree, making it clearly eligible. Most healthcare employers successfully obtain H-1B approvals for qualified physical therapists with proper credentials and state licensure.
What degree requirements do international physical therapists need for visa sponsorship?
You need a Doctor of Physical Therapy (DPT) degree from an accredited program. If your degree is from outside the U.S., you'll need a credential evaluation showing it's equivalent to a U.S. DPT. Most employers also require or prefer candidates who have completed the National Physical Therapy Examination (NPTE).
Which types of healthcare employers commonly sponsor physical therapists?
Hospital systems, rehabilitation centers, outpatient clinics, and skilled nursing facilities frequently sponsor H-1B visas for physical therapists. Large healthcare networks like HCA, Tenet Healthcare, and regional hospital systems often have established visa sponsorship processes due to ongoing staffing needs in physical therapy departments.
Do I need state licensure before applying for PT sponsorship positions?
State licensure requirements vary by employer and state. Some will sponsor candidates while they complete licensure requirements, while others prefer licensed candidates. Having licensure demonstrates commitment and removes barriers, but many employers will work with qualified candidates through the licensure process during H-1B processing.
Are there alternative visa options for physical therapists besides H-1B?
Yes, several alternatives exist. J-1 visas work for clinical training or research positions. TN visas are available for Canadian and Mexican citizens. The EB-2 NIW green card path may work for PTs with advanced training or those serving underserved populations. Some rural areas offer additional visa incentive programs.
What is the prevailing wage requirement for sponsored PT jobs?
U.S. employers sponsoring a visa must pay at least the prevailing wage, which is what workers in the same role, area, and experience level typically earn. The Department of Labor sets this rate to make sure companies aren't hiring foreign workers simply because they'd accept lower pay than a U.S. worker. It varies by job title, location, and experience. You can look up current prevailing wage rates for any occupation and location using the OFLC Wage Search page.
