SUMMARY OF POSITION
Provides person-centered assistance to clients in the development of life skills and personal goals. Coordinates client care with multidisciplinary team. Assists clients in connecting to community resources. Provides crisis intervention/prevention to clients and their families.
ESSENTIAL DUTIES & RESPONSIBILITIES
- Coordinates client care by working closely with a multidisciplinary team to develop and maintain essential elements of the client’s clinical record, including but not limited to treatment plan, progress notes, reporting and correspondence with external agents, ensuring compliance standards.
- Teaches and trains clients in activities of daily living, in group and individual settings, complying with clinical documentation requirements, and provides services according to organizational guidelines and/or protocols.
- Connects client with community support systems as determined to be appropriate, and maintains regular communications with schools, OFC’s and other family service providers that the client and their family participates with or is receiving services from.
- Provides telephone and on-site crisis intervention/prevention and follow-up to clients and their families.
- Understands how the job functions within the overall Certified Community Behavioral Health Clinics (CCBHC) model.
- Perform all required job duties and responsibilities in accordance to CCBHC Standards.

QUALIFICATIONS
Education Level
Associate's degree required. Two years of experience or two years of college coursework in related field may be substituted for degree requirement.
Years of Experience
Two years professional experience working directly with youth and families in a child welfare, school, childcare, criminal justice behavioral health or related setting preferred.
