SCHEDULE: Monday - Friday, 8hr days with variable start times of 0700 or 0800. Generally, one Nurse Care Manager is remote per week.
PURPOSE STATEMENT/POSITION SUMMARY: The Nurse Care Manager for Nephrology performs complex discharge planning for patients of different ages and developmental levels with serious illnesses. Focuses on patients with complexities related to finance and/or resource utilization. Acts as advocate for the patient/family by collaborating with the multidisciplinary team to develop and implement a case management plan to ensure quality patient outcomes, and customer satisfaction in a cost effective manner. Participates/facilitates in the development and outcome analysis of clinical pathways as a member of the multidisciplinary CQI team.
General Nephrology manages pediatric patients with various renal disease diagnosis. Our Nurse Care Managers (NCM) participate in clinic visits, patient education, triage sick calls from patients and participates in California Children Services care management, genetic testing and vaccinations.
MINIMUM QUALIFICATIONS/WORK EXPERIENCE:
- 3+ years nursing experience with advanced level pediatric clinical experience and utilization management and/or discharge planning experience.
- Thorough knowledge of managed care, utilization management, discharges planning, and continuous quality improvement concepts.
- Computer Literacy.
- Strong interpersonal and negotiation skills.
- Fluency in both English and Spanish desired.
EDUCATION/LICENSURE/CERTIFICATIONS:
- Bachelor’s Degree in Nursing required.
- Master’s degree in Nursing, Health Care Administration, or related field preferred.
- Current Registered Nurse licensure in the State of California and American Heart Association BLS/PALS required.
- CCS paneled preferred.
PAY SCALE INFORMATION: CHLA values the contribution each Team Member brings to our organization. Final determination of a successful candidate’s starting pay will vary based on a number of factors, including, but not limited to, education and experience within the job or the industry. The pay scale listed for this position is generally for candidates who meet the specified qualifications and requirements listed on this specific job description. Additional pay may be determined for those candidates who exceed these specified qualifications and requirements. We provide a competitive compensation package that recognizes your experience, credentials, and education alongside a robust benefits program to meet your needs.
POSTING RANGE: $124,488.00-$165,984.00
