Job Overview
We are seeking a dedicated and compassionate Case Manager to join our dynamic healthcare team. Duties
- Conduct thorough patient assessments to determine individual needs, including medical, social, and behavioral health factors
- Develop and implement personalized care plans in collaboration with healthcare providers, social workers, and family members
- Coordinate discharge planning to ensure safe and effective transitions from hospital to home or other care facilities
- Utilize EMR (Electronic Medical Record) systems and EHR (Electronic Health Record) systems to document patient interactions, progress notes, and care plans accurately
- Perform utilization review and management to ensure appropriate level of care while adhering to HIPAA regulations and health insurance guidelines
- Communicate effectively with patients, families, healthcare teams, and social services to facilitate understanding and engagement in the care process
- Maintain up-to-date knowledge of medical terminology, CPT coding, ICD-9/10 coding standards, and managed care policies to support accurate documentation and billing processes
Qualifications
- Proven experience in critical care settings such as ICU, emergency medicine, or Level II trauma centers is highly preferred
- Strong background in hospital medicine, acute care, or hospice care with a focus on patient service excellence
- Familiarity with social work principles and behavioral health practices for working with diverse populations
- Knowledge of medical coding (ICD-9/10), CPT coding, ICD coding standards for accurate documentation and billing
- Experience with EMR/EHR systems and proficiency in Microsoft Office applications for reporting and communication purposes
- Excellent client communication skills with the ability to work effectively with individuals from diverse cultural backgrounds
- Bilingual abilities are a plus for serving a broader patient demographic
- Certifications or experience in utilization review/management are advantageous
- Ability to handle intake processes efficiently while coordinating social services as needed for comprehensive patient support
