Duties

• Conduct thorough patient assessments to determine individual needs, including medical, social, and behavioral health factors
• Develop and implement personalized care plans in collaboration with healthcare providers, social workers, and family members
• Coordinate discharge planning to ensure safe and effective transitions from hospital to home or other care facilities
• Utilize EMR (Electronic Medical Record) systems and EHR (Electronic Health Record) systems to document patient interactions, progress notes, and care plans accurately
• Perform utilization review and management to ensure appropriate level of care while adhering to HIPAA regulations and health insurance guidelines
• Communicate effectively with patients, families, healthcare teams, and social services to facilitate understanding and engagement in the care process
• Maintain up-to-date knowledge of medical terminology, CPT coding, ICD-9/10 coding standards, and managed care policies to support accurate documentation and billing processes

Qualifications

• Proven experience in critical care settings such as ICU, emergency medicine, or Level II trauma centers is highly preferred
• Strong background in hospital medicine, acute care, or hospice care with a focus on patient service excellence
• Familiarity with social work principles and behavioral health practices for working with diverse populations
• Knowledge of medical coding (ICD-9/10), CPT coding, ICD coding standards for accurate documentation and billing
• Experience with EMR/EHR systems and proficiency in Microsoft Office applications for reporting and communication purposes
• Excellent client communication skills with the ability to work effectively with individuals from diverse cultural backgrounds
• Bilingual abilities are a plus for serving a broader patient demographic
• Certifications or experience in utilization review/management are advantageous
• Ability to handle intake processes efficiently while coordinating social services as needed for comprehensive patient support
