Responsibilities

• Deliver comprehensive patient care by assisting with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts.
• Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration to track health status accurately.
• Support medication administration following prescribed protocols while maintaining strict adherence to HIPAA regulations for patient confidentiality.
• Observe patients closely for any changes in condition or behavior, especially in dementia or Alzheimer’s care cases, and communicate findings promptly to healthcare professionals.
• Assist with specialized medical procedures such as catheter care and hospice support to ensure comfort and safety.
• Maintain detailed medical documentation within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems for accurate record-keeping and compliance.
• Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe environment for clients.
• Use medical terminology appropriately when documenting patient information and communicating with healthcare teams.
• Support patients in home and community settings by providing compassionate companionship and engaging activities tailored to individual needs.
• Follow all safety protocols during patient care activities, including CPR certification requirements and proper use of mobility aids like Hoyer lifts.

Skills

• Strong patient observation skills to detect subtle changes in health or behavior.
• Experience working in assisted living facilities, nursing homes, or long-term care environments.
• Familiarity with EMR/EHR systems for accurate medical documentation and record management.
• Expertise in assisting with ADLs such as bathing, dressing, grooming, meal prep, and light housekeeping tasks.
• Knowledge of medical terminology relevant to home health care practices.
• Ability to perform CPR and other emergency response procedures confidently.
• Experience caring for individuals with developmental disabilities or dementia-related conditions such as Alzheimer’s disease or memory care needs.
• Proficiency in medication administration following prescribed schedules and protocols.
• Skilled in working with individuals with disabilities or chronic health conditions within home or community settings.
• Competence in using medical equipment like Hoyer lifts and monitoring vital signs accurately.
