Job Description Summary:
The Care Manager works in partnership with patients, families, caregivers and providers to deliver targeted care that is organized, coordinated, cost effective and patient-centered. The Care Manager collaborates with interdisciplinary staff and stakeholders within the care continuum to promote patients’ optimal health through disease prevention and management strategies. Evidence-based interventions are applied to populations for cost efficient and outcome-based care. Outcomes include disease-based quality measures, patient satisfaction metrics, access to services, and hospital/ED utilization rates.

Bachelor's Degree (Required)
APRN - Advanced Practice Registered Nurse License - State of Ohio Board of Nursing
CPR - Cardiopulmonary Resuscitation - American Red Cross
DL - Driver's License - Department of Motor Vehicles
RN - Registered Nurse - Ohio Board of Nursing
Additional Job Description:
RN from an accredited school of Nursing with a current unencumbered RN or APP-RN licensure in the State of Ohio. Bachelor's degree and 2-5 years recent nursing practice experience with knowledge of acute hospital, community or public health nursing concepts and resources. Computer skill and data entry. Knowledge and skill in clinical assessment and disease management.
BLS issued by American Heart Association.
Associates hired after 1/1/2020 must have their Bachelor’s degree in nursing.
