Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Duchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is seeking a Full-time Patient Navigator to support our Hudson, NY site.
ROLE AND RESPONSIBILITIES
The Patient Navigator identifies individuals and/or members of their families within the target population who are in need of preventive and primary health care services and links them to the services they need. Includes care for: acute; chronic and communicable diseases; dental services; prenatal care and family planning; WIC and other nutrition services; pediatric care and immunizations. Trains and mentors the Patient Representatives. Perform other duties as assigned on as needed basis.
Essential Duties and Responsibilities:
- Consult with medical staff to link patients to services through the provision of ancillaries such as translation, transportation assistance and application assistance.
- Facilitates patient appointments within different departments of SRH and outside agencies. Follows SRH procedures for referring patients to different departments of SRH and outside agencies, utilizing SRH preferred provider lists.
- Assist patient in overcoming obstacles that would prevent them from receiving needed referrals.
- Responsible for facilitating patients to appropriate referred agencies, with specific focus on referrals to specialty care and public health insurance programs, as needed and appropriate; track progress in eCW and alert providers.
- Answer patient questions to clarify provider instructions, procedures and referral needs.
- Responsible for transmitting all necessary clinical information available to specialist for referral; obtains consultant reports; obtains consent for release of information.
- Review open referrals for upcoming medical appointments and provide to clinical team.

Minimum Education Requirement: High school diploma/ GED
Minimum Work-Related Experience: 0-2 years
Bilingual Spanish is preferred
