INTRODUCTION

Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking Full-Time Nurse Practitioner to join our team supporting The HUB site, located in Bronx, NY.
ROLE AND RESPONSIBILITIES
A Nurse Practitioner, under the direction of the of a physician, is responsible for planning, evaluating and delivering the health care of patients on assigned service. Emphases preventative health measures and education of patients. She/he will perform other duties as assigned and in compliance with systems and job specific standards.
Job type: Full-Time
Annual salary: $154,876.00 - $167,377.60
Sign-on Bonus: $3,500.00
Essential Duties and Responsibilities for a Nurse Practitioner:
- Takes histories, performs physical examinations, gives immunizations (as necessary) and does patient education and counseling.
- Orders appropriate lab work, draws blood for lab work if needed, and performs some lab tests such as wet prep microscopic exams.
- Prescribes treatment for minor illnesses, and consults with physician regarding medication, chronic illness and referrals when appropriate.
- Maintains legible and appropriate documentation and reviews records periodically. Does follow-up of abnormal pap smears and reportable STD’s.
- Attends case conferences involving patients.
- Attends continuing education programs to increase skills and knowledge.
- Attends provider meetings and participates in quality improvement programs and team assignments.
- Identifies and categorizes each patient’s age-specific grouping of needs, such as those infants, adolescent, or geriatric patients.
- Follows OSHA standards of infection control under CDC guidelines endorsed by department, organization, and overseen by Infection Control Officer.
- Provides safe and successful performance, including meeting productivity, quality and patient satisfaction standards.
MINIMUM QUALIFICATIONS

Preferred Work Experience:
3+ Years of experience as a Nurse Practitioner in Primary Care
Minimum Licensure:

- Knowledge of diagnostic and treatment procedures that nurse practitioners are trained to do and are necessary for practicing in health care centers.
- Competence in delivery of health care to patients (including preventative health education).
- Ability to collaborate with other members of the Health Center in order to maintain high quality standards and achieve client satisfaction.
