INTRODUCTION
The Intake Specialist oversees the intake and enrollment process of new Program of All-Inclusive Care for the Elderly (PACE) participants. Responsible for educating new potential participants and their caregivers on the benefits of Central Valley PACE Program. Will work independently to gather information, set appointments for assessments and document skilled needs for the incoming cases. Interacts with family caregivers and participants, providing a tour through PACE center, gathering crucial medical and social needs for new participant for enrollment processes.
Schedule availability is Monday – Friday, 8:00am – 5:00pm.
Until our Stockton location is operational, the role will be based and trained at our Modesto center.
COMPENSATION
- Compensation: $25.22 - $26.48 an hour

DUTIES AND RESPONSIBILITIES
- Provides courteous and friendly customer service in person and over the phone, giving initial encounter with PACE, demonstrating great commitment to PACE mission, gather all necessary information over the phone and answer questions while educating callers about available services.
- Conducts Central Valley PACE’s program eligibility screening, phone intake, education and enrollment assistance activities to initially determine participant needs and eligibility for enrollment.
- Responsible for understanding and documenting medical and social indications of new PACE participant to establish care needs at initial enrollment.
- Assists and oversees the enrollment of participants through the level of care (LOC) process.
- Works as part of a team to educate new potential participants and their caregivers on the benefits of Central Valley PACE Program.
- Tours participants and family caregivers at the PACE centers and expound on the service provided.
- Presents all approved cases that are enrolling to the interdisciplinary team at their centers.
- Develops and maintains relationships with potential PACE participants who call and/or email to inquire about Central Valley PACE, working to secure a face-to-face meeting with an enrollment specialist.
- Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties.
- Identify targeted areas to invite potential participants, initiate telephone inquiries and implement mail marketing campaigns.
- Collects all patient demographic information and required paperwork throughout the enrollment process within designated timeframe.
- Observes each participant for any change in physical, mental, emotional and social functioning and shall report such changes to the licensed nurse.
- Other projects and duties as assigned.
PHYSICAL DEMANDS
- Ability to lift up to 30 pounds. Moving, lifting or transferring patients may involve lifting or pushing greater than 30 pounds, should be done with assistance as appropriate.
- Must be able to hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.
- Must have vision with or without lenses that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents.
- Must have high manual dexterity.
- Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling.
WORK ENVIRONMENT
- Exposure to biohazards, including infectious material and waste and any other conditions common in a health care environment.
- Subject to unpleasant odors.
- The noise level is usually quiet to moderate, but may at times be noisy and crowded.
MINIMUM QUALIFICATIONS
- Valid California Driver’s License, acceptable driving record and vehicle insurance.
- Ability to direct responsibility for discussing and explaining the patients’ rights prior to enrollment into PACE.
- Must successfully complete the State of California DHCS PACE and Medi-Cal Marketing Certification exam within 60 days of hire.
- Must have good communication and presentation skills, both oral and written in English and Spanish.
- Bilingual: Spanish/English required.
- Marketing/Sales experience is preferred.
- Must be proficient with computer Microsoft Office suite.
DESIRED QUALIFICATIONS
- Strong ability to educate, motivate, and encourage enrollment.
- Must be a strong team player with a proven ability to ensure team coordination and communication.
- Strong technology, Electronic Medical Records and documentation experience a plus.
EDUCATION/EXPERIENCE
- Associate's Degree in Business or related field required, or two (2) years of work related experience in lieu of degree.
- Minimum one (1) year experience in social work and health care where they have conducted psychosocial assessments.
- Minimum one (1) year experience working with elderly and their families in a healthcare or community agency setting required.
- Current BLS CPR Card certified by the American Heart Association.
