Well Life Medicine, a thriving independently owned primary care and mental health clinic in Salem, Oregon, is seeking an experienced Family Nurse Practitioner to join our growing, multidisciplinary care team (at least one year experience in family medicine, urgent care or internal medicine).
We are open to part or full-time (ideally 0.8 FTE).

Job Responsibilities
- Deliver direct patient care including intakes, preventative exams, urgent care appointments and follow-up appointments
- Diagnose and treat a wide range of chronic and acute conditions across the lifespan (ages 5+)
- Conduct preventative care such as annual exams, well-child checks, well-woman visits, and PAPs
- See 16–19 patients per day with 30-minute appointments
- Create patient-centered, evidence-based treatment plans
- Foster meaningful relationships with patients and effective collaboration with medical team
- Use EHR to document encounters accurately and efficiently
- Participate in regular case reviews and coordination with Medical Director and clinical staff
- This is an on-site position.
- Minimal on-call duties, supported by after-hours answering service
Qualifications
- Licensed Family Nurse Practitioner (FNP) in Oregon or eligible to obtain within 90 days
- Spanish language fluency is eligible for pay differential!
- 1+ years of clinical experience required
- Comfortable seeing a diverse age range (5 years and up, will consider adult only for the right candidate)
- Strong time management and organizational skills
- Skilled in patient communication, clinical documentation, and teamwork
Working Hours
- Flexible options: full-time or part-time
- Tuesday–Friday day shifts only (can do 8 or 10 hour days) anytime 7-5.
- 30-minute appointment blocks, targeting 16–19 patients/day
- Work at one or both Salem locations
- No weekend or night shifts required
