Translate your credentials before applying
Foreign medical degrees and board certifications must be evaluated by a USCIS-recognized credential assessment body before PERM filing begins. Get certified translations and equivalency letters ready so your employer's attorney can build the PERM job description around your actual qualifications.
Target hospitals with active PERM filing histories
Academic medical centers, large health systems, and VA hospitals file PERM applications far more regularly than private surgical practices. Search DOL's OFLC disclosure data by employer name and occupation to identify cardiovascular surgery programs that have sponsored foreign surgeons before.
Verify your salary meets prevailing wage requirements
PERM requires your offered wage to meet or exceed the DOL prevailing wage for your occupation and geographic area. Use the OFLC Wage Search to look up the Level I through Level IV wage for your specific metro before accepting any offer, not after.
Search green-card-sponsoring roles on Migrate Mate
Migrate Mate filters cardiovascular surgery roles by employers with documented EB-2 and EB-3 sponsorship history, cutting out the guesswork. Run your search by specialty and location to surface positions where PERM sponsorship is already part of the hiring conversation.
Clarify EB-2 versus EB-3 eligibility with your employer
Cardiovascular surgeons typically qualify for EB-2 given the advanced-degree and specialty licensing requirements, but your employer's attorney sets the preference category in the I-140 petition. If your priority date matters for timing, ask explicitly which category they intend to file under.
Lock in your priority date before your visa status changes
Your PERM priority date is established when DOL receives the application, not when it's approved. If you're on a J-1 visa or H-1B visa and anticipate a status change or two-year home residency requirement, confirm your employer has initiated PERM before any gap in authorized stay occurs.
Green Card Cardiovascular Surgeon: Frequently Asked Questions
Do cardiovascular surgeons qualify for EB-2 or EB-3 green card sponsorship?
Cardiovascular surgeons qualify for EB-2 because the role requires an advanced degree, specifically a medical degree plus residency and fellowship training. EB-3 remains an option if the employer structures the PERM job description differently, but most sponsoring health systems file under EB-2 given the specialty's clear advanced-degree requirement under USCIS guidance.
How does green card sponsorship differ from H-1B for a cardiovascular surgeon?
H-1B is a temporary nonimmigrant visa capped at 65,000 per year with a lottery. Green card sponsorship through PERM and I-140 leads to permanent residency with no annual cap at the EB-3 level for most countries. The PERM process is longer, often 18 to 36 months from filing to approval, but the outcome is permanent rather than renewable status tied to a single employer.
Which employers are most likely to sponsor a cardiovascular surgeon for a green card?
Academic medical centers, large regional health systems, and government-affiliated hospitals are the most consistent PERM sponsors for cardiovascular surgeons. These organizations have dedicated immigration counsel and established sponsorship pipelines. Private surgical groups and smaller specialty practices rarely initiate PERM because of the cost and administrative burden involved.
How can I find cardiovascular surgery jobs that include green card sponsorship?
Migrate Mate lets you filter cardiovascular surgery and cardiothoracic roles specifically by employers with EB-2 and EB-3 sponsorship histories, so you're not cold-applying to positions that don't sponsor. Standard job searches don't surface this information, which means most foreign surgeons waste time on applications that will never lead to a PERM filing.
Does the PERM process require a cardiovascular surgeon to have U.S. board certification?
PERM itself doesn't mandate U.S. board certification, but the job requirements in your PERM application must reflect what the employer genuinely requires for the position. Most U.S. hospitals require or strongly prefer board eligibility or certification from the American Board of Thoracic Surgery, so that credential effectively becomes part of the PERM minimum requirements in practice.
