Remote Behavioral Health Case Manager (Licensed)

What You’ll Do
- Conduct comprehensive clinical assessments integrating medical, behavioral health, and psychosocial needs
- Develop, implement, and monitor individualized care plans in collaboration with members and providers
- Coordinate care with medical and behavioral health professionals to ensure high‑quality, cost‑effective outcomes
- Provide crisis intervention, timely referrals, and follow‑up support when clinically indicated
- Use motivational interviewing and coaching to engage members and promote sustainable health behavior changes
- Identify quality of care concerns and escalate appropriately
- Maintain accurate, timely documentation in compliance with regulatory and accreditation standards
What You Bring - Required
- Master’s degree in social work or counseling
- Active, unrestricted independent clinical license in your state (LCSW Licensed Clinical Social Worker, LSW Licensed Social Worker, LISW Licensed Independent Social Worker, LCPC Licensed Clinical Professional Counselor, LPC Licensed Clinical Professional, LP Licensed Psychologist, LMFT Licensed Marriage and Family Therapist, LMHC Licensed Mental Health Counselor)
- 3+ years of post‑master’s direct clinical experience
- 3+ years of strong knowledge of mental health and substance use disorders
- 3+ years of experience with crisis intervention and motivational interviewing
- 3+ years of case management experience
- Ability to multitask in a fast‑paced, telephonic work environment with strong technical skills
Preferred
- Managed care or utilization review experience
- Discharge planning or care coordination experience

$54,095.00 - $116,760.00
This pay range represents the base hourly rate or base annual full‑time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short‑term incentive program in addition to the base pay range listed above.
