Valley Professionals Community Health Center is hiring a Licensed Clinical Social Worker or a Licensed Mental Health Counselor for our North Terre Haute and South Terre Haute locations.

JOB DUTIES
This position would be responsible for providing professional behavioral healthcare to VPCHC patients and maintaining the related patient documentation. In addition, this person would coordinate patient treatment with a multidisciplinary team to assure continuity of care and the comprehensiveness of health care plans.
REQUIREMENTS
- Current Indiana LSW, LCSW, LMHC, LMHCA, LCAC licensure
- Previous background and training in addictions counseling preferred
