Community Outreach & Marketing
- Develop and maintain relationships with hospitals, physician offices, rehabilitation centers, assisted living facilities, senior centers, social workers, case managers, and other referral sources.
- Represent the agency at networking events, health fairs, community meetings, and educational presentations.
- Promote agency services to prospective clients, referral partners, and community organizations.
- Develop marketing strategies to increase community awareness and generate new referrals.
- Deliver marketing materials and maintain regular contact with referral sources.
- Identify opportunities to expand the agency's presence within the community.
- Track outreach efforts and referral activity to measure effectiveness.
Client Care Coordination
- Meet with prospective clients and families to explain agency services and answer questions.
