SYNERGY HomeCare of Hurst is growing, and we are looking for a driven, people-focused Sales Representative / Community Liaison to help expand our presence throughout the community and build strong referral partnerships.
This is an ideal role for someone with experience in home care, home health, hospice, senior living, medical sales, healthcare staffing, or community outreach — or an experienced salesperson who is confident building relationships and generating new business.
At SYNERGY HomeCare, we provide compassionate non-medical care that helps seniors and adults remain safe, independent, and comfortable in their own homes. We are looking for someone who believes in relationship-based sales and wants to make a meaningful impact while growing a successful territory.
ROLE AND RESPONSIBILITIES

Build and maintain referral relationships with:

Hospitals and case managers
- Rehab and skilled nursing facilities
- Assisted living and senior communities
- Home health and hospice agencies
- Physicians’ offices and healthcare professionals

Community organizations and senior resources

Conduct daily in-person sales and marketing visits
- Generate new referral partnerships and client opportunities
- Develop and execute a territory growth plan
- Attend networking events, health fairs, and community outreach opportunities
- Represent SYNERGY HomeCare professionally within the healthcare community
- Track activity and maintain referral relationships through CRM systems
BASIC QUALIFICATIONS
- Experience in healthcare sales, home care, home health, hospice, senior services, staffing, or relationship-based business development preferred but not required.
- Comfortable with cold calling and face-to-face relationship building
- Strong communication and presentation skills
- Self-motivated and able to work independently
- Organized, professional, and goal-oriented
- Valid driver’s license and reliable transportation
