Verify your credentials meet ECFMG standards
Before applying, confirm your medical degree and USMLE scores are ECFMG-certified. PERM petitions for physician roles require documented foreign credential equivalency, and gaps here are the most common reason employers abandon sponsorship mid-process.
Target employers with J-1 waiver experience
Hospitals and health systems that have previously sponsored J-1 visa Conrad 30 waivers already understand physician immigration pathways. These employers are far more likely to fund PERM filings and tolerate the multi-year timeline than facilities with no prior foreign physician hires.
Search green card sponsoring employers on Migrate Mate
Use Migrate Mate to filter Medical Doctor roles by employers with active EB-2 and EB-3 PERM filing history. This cuts your research time significantly and surfaces health systems already set up to sponsor, rather than educating a new employer from scratch.
Understand the PERM prevailing wage before negotiating
Your employer must pay at least the DOL-certified prevailing wage for your specialty and location. Look up the wage level for your SOC code and region using the OFLC Wage Search before salary discussions so you can confirm the offer will satisfy PERM requirements.
Ask employers about National Interest Waiver eligibility
Physicians who commit to practicing in underserved or HPSA-designated areas can self-petition for an EB-2 National Interest Waiver, bypassing PERM entirely. If your target role is in a shortage area, confirm with USCIS guidance whether this route lets you avoid employer-dependent labor certification.
Clarify the sponsorship scope in your employment contract
Some hospitals sponsor PERM but exclude I-140 premium processing or cap their legal fee contributions. Before signing, get written confirmation that sponsorship covers PERM filing, I-140 petition, and adjustment of status fees through the full green card process.
Green Card Medical Doctor: Frequently Asked Questions
Does a Medical Doctor role qualify for EB-2 or EB-3 green card sponsorship?
Most physician roles qualify for EB-2 because they require an advanced degree, specifically an MD or DO, which meets the advanced-degree standard directly. EB-3 is also available for licensed physicians and covers the same PERM labor certification process. The EB-2 category typically applies to the majority of clinical and research physician positions at U.S. health systems.
How does the PERM green card process differ from H-1B sponsorship for physicians?
H-1B visa is a temporary work visa capped at 85,000 annually with a lottery, while PERM-based EB-2 and EB-3 sponsorship leads to permanent residency with no annual lottery. PERM requires your employer to conduct DOL-supervised recruitment and certify no qualified U.S. workers were available. The total timeline from PERM filing to green card approval typically runs two to four years for most nationalities, though Indian and Chinese nationals face longer backlogs due to per-country caps.
Where can I find Medical Doctor jobs where the employer already sponsors green cards?
Use Migrate Mate to search Medical Doctor roles filtered by employers with documented EB-2 and EB-3 PERM filing history. Employers with prior PERM experience for physician roles have already built internal processes for sponsorship, which significantly reduces the risk of the process stalling after you accept an offer.
Can a physician self-petition for a green card without an employer sponsor?
Yes, physicians who agree to work full-time in a federally designated Health Professional Shortage Area or Medically Underserved Area for at least five years can self-petition for an EB-2 National Interest Waiver through USCIS. This route bypasses the PERM labor certification process entirely, meaning you don't need an employer to initiate or fund the filing. Qualifying service must be completed after approval.
What happens to my green card sponsorship if I change hospital employers before the process is complete?
If you change employers before your I-140 is approved, your PERM certification is tied to the original employer and generally cannot transfer. However, once your I-140 has been approved for at least 180 days and your priority date is current or near current, portability rules under AC21 allow you to move to a similar physician role at a new employer without restarting the process from PERM.
