Green Card Neonatologist Jobs
Neonatologist roles qualify for EB-2 and EB-3 green card sponsorship through the PERM labor certification process, which requires employers to document that no qualified U.S. workers are available before filing your I-140 immigrant petition. Hospitals and academic medical centers routinely sponsor fellowship-trained neonatologists for permanent residency.

Verify your ECFMG certification is current
PERM petitions for neonatologists require proof of U.S. licensure eligibility. Confirm your ECFMG certification and state medical license are in order before any employer begins the DOL labor certification process, or the application stalls.
Target academic medical centers with NICU programs
Level III and Level IV NICUs at teaching hospitals file PERM petitions more frequently than community hospitals. Filter your search by institutions with active neonatology fellowship programs, since those employers already have sponsorship infrastructure and legal counsel in place.
Search open EB-2 and EB-3 roles on Migrate Mate
Migrate Mate surfaces neonatologist positions where employers have verified green card sponsorship history. Use it to identify which hospital systems are actively filing EB-2 or EB-3 petitions for physician roles rather than spending months on cold outreach.
Understand how prevailing wage affects your offer
Before signing an employment contract, check the OFLC Wage Search to confirm the offered salary meets the DOL prevailing wage for your SOC code and geographic area. A wage below the certified level triggers a PERM denial regardless of employer intent.
Ask about EB-2 NIW eligibility at the offer stage
Fellowship-trained neonatologists, particularly those in underserved regions or research-focused roles, sometimes qualify for an EB-2 National Interest Waiver, which eliminates the PERM requirement entirely and lets you self-petition through USCIS without employer-driven labor certification.
Clarify who covers PERM legal and filing costs
Negotiate attorney fees and PERM advertising costs before accepting an offer. DOL regulations prohibit employers from passing certain PERM costs to you, but I-140 premium processing fees and adjustment-of-status costs can shift between parties depending on your employment agreement.
Green Card Neonatologist: Frequently Asked Questions
Does a neonatologist role qualify for EB-2 or EB-3 sponsorship?
Neonatology qualifies for both categories. EB-2 covers advanced-degree professionals, and neonatologists hold an MD plus fellowship training, which satisfies that standard. EB-3 is available for professional and skilled worker roles. Most hospital sponsors file under EB-2, though EB-3 is an option when EB-2 backlogs affect your country of birth.
How does green card sponsorship differ from H-1B for a neonatologist?
H-1B visa sponsorship is temporary, subject to annual caps and lottery selection, and must be renewed every few years. PERM-based green card sponsorship has no annual cap at the petition stage, leads to permanent residency, and lets you begin building your priority date while on H-1B status. The tradeoff is a longer overall timeline, often three to five years or more depending on your country of birth.
How long does the PERM process take for a physician?
DOL currently processes standard PERM applications in twelve to eighteen months, though supervised recruitment timelines vary. After PERM certification, your employer files the I-140 with USCIS. If your priority date is current, you can then file for adjustment of status or consular processing. For most neonatologists born in high-demand countries, the wait for an available visa number adds additional years.
How do I find neonatologist jobs that sponsor green cards?
Most hospital job postings don't explicitly state green card sponsorship, so filtering by institutions with documented PERM filing history saves significant time. Migrate Mate lets you search specifically for neonatologist roles where employers have verified employment-based sponsorship, removing the guesswork from identifying which health systems will initiate the PERM process for physician candidates.
Can a neonatologist self-petition for a green card without employer sponsorship?
Yes, through the EB-2 National Interest Waiver. USCIS allows physicians who commit to practicing full-time in a federally designated underserved area for at least five years to self-petition without going through PERM. This pathway bypasses employer-driven labor certification entirely, which is particularly valuable if you're building your own practice or working in a rural or shortage-area facility.
