A Licensed Clinical Social Worker (LCSW) contributes to AHF by blending the organizational mission with the provision of care management services. The LCSW provides preventive health services to prevent disease, disability, and other health conditions or their progression; to prolong life; and promote physical and mental health.
A LCSW enhances the reputation of AHF by impacting improved member health outcomes and health equity which leads to longer, healthier, and happier lives. The LCSW assists members in their homes, community, or clinic setting, provides support of adherence and compliance to care, and helps members identify socio-economic issues that affect their overall health.

ROLE AND RESPONSIBILITIES
The LCSW will work as a part of a close-knit team to provide complex care management and delivers culturally and linguistically responsive support services to members within the HIV/AIDS Special Needs population enrolled in a Medicare/Medi-Cal health plan. The LCSW applies advanced clinical practice skills to support the Care Team by identifying, cataloging, and coordinating community resources, and acts as a liaison to address the psychosocial needs of members. The LCSW actively participates as a member of the interdisciplinary team, contributing to assessment, data gathering, and care planning. They also participate in CMS, DHCS/DMHC, and other relevant committees to strengthen organizational understanding of the social determinants of health affecting the members served by the plan, and consistently go the extra mile to ensure members are informed about and have access to their health plan and community benefits. The LCSW addresses psychosocial barriers that impact the management, control, or prevention of chronic and infectious diseases; mental health conditions and substance use disorders; need for preventive services; perinatal and reproductive health needs; environmental and climate-related health challenges; oral health; aging; injury; and domestic or interpersonal violence. The LCSW demonstrates leadership by serving as a mentor to Community Health Workers (CHWs). The LCSW assists members in crisis, manages referrals, and oversees behavioral health transitions of care in collaboration with the Utilization Management Department and Care Team Managers. They lead behavioral health meetings, maintain tracking of all referrals, and regularly update leadership on successful linkages to care. Additionally, the LCSW updates care plans and ensures timely, accurate documentation consistent with organizational policies, accreditation standards, and regulatory requirements.
BASIC QUALIFICATIONS
- Licenses and Certifications: Masters of Social Work from an accredited school. CA Licensed Clinical Social Worker (LCSW)
- At least two (2) years of social work experience in Managed Care is preferred.
- Bilingual Spanish preferred.
