Verify your credentials meet specialty occupation
USCIS requires a direct relationship between your degree and the GI NP role. An MSN or DNP with a focus in adult-gerontology or family practice typically satisfies this. A general BSN alone may draw an RFE, so confirm your transcript supports the specialty.
Target cap-exempt employers first
Hospitals affiliated with universities or nonprofit research centers can file H-1B petitions outside the annual lottery cap. GI divisions within academic medical centers are a practical starting point if you want to avoid cap-season timing constraints entirely.
Search LCA filing history by occupation code
Use Migrate Mate to filter employers by verified H-1B LCA filing history for NP roles. This surfaces which GI practices and health systems have actively sponsored similar positions, so you're targeting employers already familiar with the process.
Confirm prevailing wage before accepting an offer
Your employer's LCA must certify a wage at or above the DOL prevailing wage for your metro area and experience level. Run the OFLC Wage Search for SOC code 29-1171 before negotiating to know whether the offer clears the required threshold.
Ask employers about H-1B filing timelines early
Cap-subject H-1B petitions must be filed in early April for an October 1 start. If a GI practice has never sponsored before, they need lead time to retain counsel and complete the LCA. Raise the timeline in the second or third interview round, not after an offer.
Document procedural competencies for the I-129 petition
USCIS may request evidence that your role requires specialty-level knowledge. Compile records of GI-specific training: endoscopy assist competencies, Barrett's surveillance protocols, or IBD management certifications. These strengthen the specialty occupation argument beyond the degree alone.

Find Gastroenterology Nurse Practitioner JobsGastroenterology Nurse Practitioner H-1B Visa: Frequently Asked Questions
Does a Gastroenterology Nurse Practitioner role qualify as a specialty occupation for H-1B purposes?
Yes, provided the position requires a master's or doctoral degree in a specific nursing specialty. USCIS evaluates whether the role's duties demand graduate-level knowledge, not just any bachelor's degree. GI NP positions that require independent diagnostic and procedural decision-making generally meet this standard, but your employer's job description must clearly reflect those advanced requirements.
Which types of employers most commonly sponsor H-1B visas for GI Nurse Practitioners?
Academic medical centers, large hospital systems, and multispecialty group practices with dedicated GI departments are the most active H-1B sponsors for this role. Smaller independent GI practices can sponsor too but may lack prior experience with the process. You can find employers with verified H-1B LCA filing history for NP roles directly on Migrate Mate, which filters by occupation code and employer.
Can I change employers on H-1B status if I'm already working as a GI Nurse Practitioner?
Yes. Under AC21 portability, you can move to a new employer once your I-140 has been approved and your I-485 has been pending for 180 days or more, provided the new role is in the same or a substantially similar occupational classification. For an H-1B transfer without a pending green card, your new employer simply files an H-1B transfer petition before you start the new role.
How does the H-1B prevailing wage requirement affect GI NP job offers?
Your employer's Labor Condition Application must certify they'll pay at least the DOL prevailing wage for your SOC code and geographic area. There are four wage levels tied to experience and supervision, so a senior GI NP role will have a higher floor than an entry-level position. Reviewing the OFLC Wage Search before negotiating helps you confirm the offer is LCA-compliant before the employer begins filing.
Is a Gastroenterology NP role subject to the H-1B annual cap and lottery?
Most private GI practices and for-profit health systems are subject to the annual cap, which means registration in the lottery held each March for an October 1 start date. However, if the employer is a nonprofit hospital, a university-affiliated health system, or a qualifying research institution, they may be cap-exempt and can file at any time of year without lottery registration.
