Verify your ECFMG and fellowship credentials early
USCIS scrutinizes MFM petitions closely because the subspecialty requires both residency and fellowship documentation. Gather your ECFMG certificate, ABOG board eligibility letter, and fellowship completion records before applying to any position.
Target cap-exempt academic medical centers
University-affiliated hospitals and nonprofit research institutions file H-1B petitions outside the annual cap, so there's no lottery risk. Filter your search on Migrate Mate by employers with verified H-1B LCA filing history for physician roles.
Research prevailing wage levels before negotiating
Your employer's LCA must certify a wage at or above the DOL prevailing wage for your metro area and SOC code. Use the OFLC Wage Search to look up Level III and Level IV wage rates for maternal-fetal medicine before your offer discussion.
Confirm the employer holds active DEA and state medical staff privileges
USCIS requires that your H-1B petition reflect a valid, licensed practice environment. Delays in your state medical license or hospital credentialing can stall the I-129 filing, so align your credentialing timeline with the employer's petition preparation window.
Ask whether premium processing is included in the offer
USCIS offers premium processing for H-1B petitions, reducing adjudication to 15 business days. Many MFM employers absorb this cost, but it's not automatic. Clarify who pays before you sign, since clinical start dates in academic medicine are often semester-aligned and inflexible.
Review the O*NET profile to strengthen your specialty occupation argument
The O*NET occupation profile for maternal-fetal medicine physicians documents the degree requirements and specialized knowledge USCIS uses to evaluate specialty occupation status. Cross-reference your job duties against it when your attorney drafts the support letter.

Find Maternal-Fetal Medicine Physician JobsMaternal-Fetal Medicine Physician H-1B Visa: Frequently Asked Questions
Does a Maternal-Fetal Medicine Physician role qualify as a specialty occupation for H-1B purposes?
Yes. MFM physicians hold a medical degree plus subspecialty fellowship training in maternal-fetal medicine, which satisfies USCIS's requirement that the position normally require at least a bachelor's degree in a specific specialty. The O*NET profile and ABOG subspecialty board standards both support this classification, making denial on specialty occupation grounds uncommon for properly documented petitions.
Which types of employers typically sponsor H-1B visas for MFM physicians?
Academic medical centers, children's hospitals with high-risk obstetric units, and large regional health systems are the most active sponsors. Many of these are cap-exempt because of their nonprofit or university affiliation, meaning they can file year-round without waiting for the annual H-1B lottery. Use Migrate Mate to filter for employers with confirmed LCA filing history in maternal-fetal medicine and high-risk obstetrics.
How does board certification affect my H-1B petition as an MFM physician?
ABOG board eligibility or certification strengthens your petition by documenting subspecialty training beyond general obstetrics. While USCIS doesn't require board certification to approve a specialty occupation petition, employers often make it a condition of the offer letter. If you're still completing your eligibility window, include your fellowship completion letter and program director verification in the petition package.
Can I transfer my H-1B to a new MFM employer mid-status?
Yes, through H-1B portability under AC21. Once your current petition has been approved and you've been in valid H-1B status for at least 180 days after filing, you can begin working for the new employer as soon as the transfer petition is filed, without waiting for approval. Your new employer must file a new I-129 and LCA before your start date.
What prevailing wage level should I expect for an H-1B MFM position?
Most MFM positions are classified at DOL wage Level III or Level IV, reflecting the subspecialty fellowship requirement and the clinical complexity of the role. Your employer's LCA must certify a wage at or above the applicable level for your geographic area and SOC code. Use the OFLC Wage Search to look up current figures for your metro before the offer stage.
