H-1B Visa Family Practice Physician Jobs
Family Practice Physicians are among the most consistently sponsored H-1B visa roles in the U.S., driven by persistent primary care shortages in rural and underserved communities. Employers file LCAs through DOL before petitioning USCIS, and most sponsoring health systems expect board eligibility or certification alongside an unrestricted state medical license.

Get Access To All JobsTips for Finding H-1B Visa Sponsorship as a Family Practice Physician
Verify your ECFMG certification before applying
International medical graduates need active ECFMG certification before any U.S. employer can file your H-1B petition. Confirm your certification status and ensure your medical school credentials are fully verified, delays here stall the entire sponsorship timeline.
Target HPSA-designated employers for faster sponsorship
Health systems operating in Health Professional Shortage Areas sponsor H-1B physicians at higher rates and often absorb more filing costs. Search for federally qualified health centers and rural health clinics, which face chronic primary care gaps and file regularly for family medicine physicians.
Check the prevailing wage before negotiating your offer
Your employer's LCA must certify a wage at or above the DOL prevailing wage for your role and location. Run your job title and county through the OFLC Wage Search before your offer conversation so you understand the wage floor your employer is already obligated to meet.
Use Migrate Mate to find employers with H-1B filing history
Targeting employers who've sponsored family medicine physicians before eliminates guesswork. Migrate Mate filters jobs by verified H-1B LCA filing history, so you're applying to health systems that have already navigated the sponsorship process for your occupation.
Ask whether the employer requires J-1 waiver coordination
Some family physicians on J-1 visa exchange visitor status pursue Conrad 30 or federal agency waivers before transitioning to H-1B. If you hold or held J-1 status, confirm with your prospective employer whether the waiver service obligation affects their H-1B filing timeline or practice location restrictions.
Confirm the I-129 filing window relative to your start date
USCIS accepts H-1B cap-exempt petitions year-round, but your employer needs time to obtain LCA certification from DOL first, which runs up to seven business days. Align your expected start date with your employer's HR and legal team to avoid gaps between your current authorization and H-1B approval.
H-1B Visa Family Practice Physician: Frequently Asked Questions
Do Family Practice Physician roles qualify as H-1B specialty occupations?
Yes. Family practice medicine requires a specific body of theoretical and practical knowledge attained through a U.S. medical degree or its foreign equivalent, which satisfies USCIS's specialty occupation definition. You'll need to hold an MD or DO degree, pass USMLE steps, and maintain an unrestricted state medical license. ECFMG certification is required for international medical graduates.
Are hospitals and health clinics cap-exempt for H-1B sponsorship?
Many are. Nonprofit hospitals affiliated with a university or research institution, federally qualified health centers, and government-owned health systems often qualify as cap-exempt employers. Cap-exempt status means your employer can file your H-1B petition at any time of year without waiting for the April lottery window, which significantly speeds up your ability to start.
How do I find Family Practice Physician employers who actively sponsor H-1B visas?
Migrate Mate is the most direct way to find family medicine roles at employers with documented H-1B LCA filing history. You can filter by occupation and verify that the employer has filed for physicians in your specialty before, rather than relying on job postings that don't disclose sponsorship willingness upfront.
What happens to my H-1B status if I change health systems after sponsorship?
Your H-1B is employer-specific, so changing employers requires your new health system to file a new H-1B petition before or concurrent with your start date. Under AC21 portability rules, if your original petition has been pending for 180 days or more, you may be able to port to a new employer in the same or similar occupational classification while the petition is adjudicated.
Does a J-1 waiver affect my path to H-1B sponsorship as a family physician?
It can. If you're completing a J-1 service obligation through a Conrad 30 waiver, your H-1B sponsorship is typically tied to the waiver employer for the three-year service period. After fulfilling the obligation, you're free to pursue standard H-1B sponsorship with any qualifying employer. Coordinate the transition timeline carefully to avoid unauthorized status between your waiver completion and H-1B approval.
