Verify your degree meets specialty occupation
USCIS requires your degree field to directly relate to orthopedic physical therapy. A foreign PT degree must be evaluated for U.S. equivalency before your employer files. A credential evaluation from a FCCPT-recognized agency is the standard employers expect.
Search LCA filings by SOC code
Use the OFLC Wage Search to pull Labor Condition Application filings under SOC 29-1123. Employers who filed LCAs in the past two years are actively sponsoring PTs and are far more likely to do it again for your hire.
Target employers with in-house HR infrastructure
Hospital systems and large outpatient PT groups have HR teams experienced with immigration filings. Smaller single-location clinics often won't sponsor because they lack the infrastructure, so concentrate your search on multi-site employers.
Find H-1B-sponsoring orthopedic employers on Migrate Mate
Search Migrate Mate to filter orthopedic PT roles at employers with verified H-1B LCA filing history. This surfaces active sponsors so you spend your time on applications that can actually lead to a petition, not rejections over immigration policy.
Clarify state licensure timing with your employer
H-1B petitions for physical therapists require a valid state PT license before USCIS approves the petition. Confirm with your prospective employer which state license they need and whether they'll support licensure exam costs before you accept an offer.
Account for cap-subject filing deadlines in your timeline
Most orthopedic PT employers are cap-subject, meaning your H-1B start date can only be October 1. Employers must file by April 1. Structure your job search so you have an offer in hand well before the March registration window opens.
H-1B Visa Orthopedic Physical Therapist: Frequently Asked Questions
Does an orthopedic physical therapist role qualify as an H-1B specialty occupation?
Yes. USCIS classifies physical therapy as a specialty occupation because the role normally requires at least a Doctor of Physical Therapy (DPT) or a foreign equivalent bachelor's or master's degree in physical therapy. Your employer will document this in the I-129 petition, and your degree transcript and credential evaluation will support the specialty occupation claim.
Do I need a U.S. state PT license before my employer files the H-1B petition?
Yes, in most cases. USCIS expects that an H-1B beneficiary is fully qualified to perform the job on the petition start date, and practicing physical therapy in any U.S. state requires a valid state license. Your employer should confirm which state license applies and build the credential evaluation and licensure timeline into the petition filing schedule.
Which types of employers typically sponsor H-1B visas for orthopedic physical therapists?
Hospital systems, large outpatient orthopedic and sports medicine groups, rehabilitation chains, and university medical centers are the most consistent H-1B sponsors for this role. These employers have HR and legal infrastructure to manage LCA filings and USCIS petitions. You can filter for employers with verified H-1B LCA filing history on Migrate Mate.
How does the H-1B prevailing wage requirement affect orthopedic PT offers?
Your employer must pay you at least the prevailing wage for SOC code 29-1123 in the specific metropolitan area where you'll work, as certified in the Labor Condition Application filed with DOL. The prevailing wage is set at either Level I or Level II depending on your experience. You can look up the applicable wage using the OFLC Wage Search before evaluating any offer.
Can an orthopedic PT clinic sponsor me if they've never filed an H-1B before?
A first-time sponsor can file, but it requires more preparation. The employer must register with USCIS, file an LCA with DOL, and submit an I-129 petition with supporting documentation. Smaller single-location clinics often decline because the process is resource-intensive. If a clinic is willing but inexperienced, they'll need an immigration attorney to manage the filing correctly.
