Verify your ECFMG certification before applying
Foreign medical graduates must hold valid ECFMG certification before a U.S. employer can file your H-1B petition. Confirm your certification status is current and matches the name on your passport to avoid filing delays.
Target cap-exempt hospitals to skip the lottery
Nonprofit hospitals, university health systems, and federally qualified health centers can file H-1B petitions year-round without lottery exposure. Prioritizing these employers gives you a direct path to approval without waiting for the annual cap season.
Search LCA filing history on Migrate Mate
Before applying, use Migrate Mate to check which hospital systems have active H-1B Labor Condition Applications for hospitalist roles. This shows you which employers are actively sponsoring, not just those who claim they might.
Confirm prevailing wage level before negotiating
Your employer's LCA must certify a wage at or above the DOL prevailing wage for your location and SOC code. Use the OFLC Wage Search to look up Level I through Level IV wages for hospitalists in your target metro before your offer conversation.
Ask about premium processing during offer stage
USCIS premium processing reduces adjudication to 15 business days. For hospitalist roles with credentialing timelines, confirm upfront whether your employer will pay for premium processing so your start date doesn't slip past your expected onboarding window.
Understand how state medical licensure affects your timeline
Most states require a valid medical license before you can work as a hospitalist, and some license applications take three to six months. Start your state licensure application as soon as you have a conditional offer, parallel to your employer's H-1B filing.

Find Hospitalist JobsHospitalist H-1B Visa: Frequently Asked Questions
Does a hospitalist role qualify as an H-1B specialty occupation?
Yes. USCIS classifies hospitalist positions as specialty occupations because they require at minimum a Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree, or a foreign equivalent recognized by ECFMG. The direct tie between a specific advanced degree and the job duties satisfies the specialty occupation standard without additional documentation.
Which types of employers sponsor H-1B visas for hospitalists?
Hospital medicine groups employed by nonprofit health systems, academic medical centers, and Veterans Affairs facilities are the most common H-1B sponsors for hospitalists. These organizations frequently qualify as cap-exempt filers, meaning they can submit petitions outside the annual lottery window. You can browse employers with verified H-1B filing histories for hospitalist roles on Migrate Mate.
Can a hospitalist H-1B petition be filed cap-exempt?
It depends on the employer, not the role. If the hospital or medical group is a nonprofit entity affiliated with an institution of higher education, or is itself a nonprofit research organization, it qualifies for cap-exempt filing. Cap-exempt employers can file H-1B petitions at any time of year and are not subject to the 65,000 general cap or the 20,000 U.S. master's exemption.
How does the DOL prevailing wage requirement work for hospitalist H-1B filings?
Before filing your H-1B petition, your employer must submit an LCA to DOL certifying that your offered wage meets or exceeds the prevailing wage for your occupational classification and work location. Prevailing wages are organized into four levels based on experience and job complexity. Use the OFLC Wage Search to look up the current wage levels for hospitalists in your target city before you receive a formal offer.
What happens to my H-1B status if I change hospital employers?
You can transfer your H-1B to a new employer through H-1B portability under AC21, which allows you to begin working for the new employer once they file a new H-1B petition on your behalf, as long as your original petition had been pending or approved for at least 180 days. Your new employer must file before your current authorized stay expires, and the new role must still qualify as a specialty occupation.
