H-1B Visa Neonatologist Jobs

Academic medical centers, children's hospitals, and large hospital systems are the most active H-1B visa sponsors for this subspecialty, with many filing petitions well before a physician's J-1 visa or residency authorization expires.

Verify your ECFMG certification before applying
International medical graduates must hold active ECFMG certification before any U.S. employer can file an H-1B petition on your behalf. Confirm your certification status is current and matches the name on your passport to avoid delays at the USCIS filing stage.
Search LCA filings for neonatology-specific roles
Use the OFLC Wage Search to filter Labor Condition Application filings by SOC code 29-1062 (Family and General Practice Physicians is a common mis-filing; 29-1228 covers Physicians, All Other). Employers who have filed LCAs for neonatologist titles in the past two years are your highest-probability targets.
Target hospitals with J-1 waiver Conrad 30 history
Many neonatologists enter on J-1 exchange visitor visas and need a Conrad 30 waiver to change status. Hospitals that routinely sponsor Conrad 30 waivers already have immigration counsel experienced with physician H-1B filings, making the transition smoother and faster.
Use Migrate Mate to find verified H-1B sponsors
Filter neonatologist openings by employers with confirmed H-1B LCA filing history on Migrate Mate. This surfaces hospital systems and academic medical centers that have sponsored physicians in this subspecialty, letting you skip employers unlikely to file.
Negotiate premium processing into your offer terms
Standard H-1B adjudication can take several months, which creates a gap if your current authorization expires before approval. Request that your prospective employer file with USCIS premium processing as a condition of your offer, compressing the review window to 15 business days.
Understand cap-exempt hospital filing advantages
Neonatologists employed directly by nonprofit hospitals affiliated with research institutions or universities may qualify for cap-exempt H-1B petitions, bypassing the annual lottery entirely. Confirm the employer's cap-exempt status with their legal team before accepting an offer.
H-1B Visa Neonatologist: Frequently Asked Questions
Does a neonatologist role qualify as a specialty occupation for H-1B purposes?
Yes. Neonatology requires a medical degree, pediatric residency, and a subspecialty fellowship, satisfying USCIS's specialty occupation standard that the position normally requires at minimum a bachelor's degree in a specific field. The highly specialized medical training required means these petitions face minimal scrutiny on specialty occupation grounds compared to many other H-1B roles.
Are most neonatologist H-1B positions subject to the annual cap and lottery?
Not always. Many neonatologist positions are at nonprofit academic medical centers or teaching hospitals that qualify as cap-exempt employers under USCIS rules. Cap-exempt petitions can be filed at any time without lottery selection. If the hospital is a for-profit entity with no qualifying nonprofit affiliation, the standard cap and lottery apply. Confirm the employer's status before signing an offer.
How do I find neonatologist jobs at employers that actively sponsor H-1B visas?
Search on Migrate Mate, which filters job listings by verified H-1B LCA filing history so you can see which hospital systems and academic medical centers have sponsored physicians in this subspecialty. You can also cross-reference the OFLC Wage Search using SOC code data to identify employers who have filed certified LCAs for neonatology or closely related physician titles within the past two years.
Can I transition from a J-1 exchange visitor visa to an H-1B as a neonatologist?
Yes, but J-1 physicians are generally subject to a two-year home-country physical presence requirement before changing status. A Conrad 30 waiver through a state health department or a federal agency waiver can eliminate this requirement, allowing you to change to H-1B status without returning home. Your sponsoring hospital's immigration counsel typically manages this process, and hospitals with a history of Conrad 30 filings are better positioned to guide you through it.
What does the H-1B prevailing wage requirement mean for neonatologist compensation?
Before filing an H-1B petition for a neonatologist, your employer must file a Labor Condition Application certifying they'll pay at least the DOL prevailing wage for the role in the specific geographic area. Prevailing wages are determined by skill level (Level I through IV) and are publicly searchable through the OFLC Wage Search. In practice, board-certified neonatologists typically command compensation well above the Level I threshold, so most academic and hospital employers file at Level III or IV.
